Advance Care Planning
Advance care planning (ACP) is a communication process that aims to help an individual plan for future medical care if the person is unable to make his or her own medical decisions. An estimated 45–70% of older adults facing end-of-life (EOL) treatment decisions are incapable of making those decisions themselves (Institute of Medicine [IOM] 2014). ACP helps an individual prepare for a sudden unexpected illness or injury, from which the person may recover, as well as the dying process and ultimately death.
