Psycho-oncologie: op het scherp van de snede

Samenvatting

Kanker is tegenwoordig een van de eerste doodsoorzaken in de westerse samenleving, maar tevens overleven veel mensen hun ziekte, vanwege sterk verbeterde medische behandelmogelijkheden. De afgelopen dertig jaar heeft de psycho-oncologie zich ontwikkeld van een on(der)belicht gebied tot een erkend, belangrijk aspect van kankerrehabilitatie. Aanpassing aan de ziekte kanker vergt vaak een complex psychisch proces, waarin dagelijkse beslommeringen en existentiële zorgen hand in hand gaan. Afhankelijk van de ziektefase waarin iemand zich bevindt, kan men worstelen met verlies en/of angst. Sommige mensen hebben hierbij behoefte aan professionele psychische hulp. In zo’n geval is het belangrijk dat de psycholoog/psychotherapeut beschikt over passende kennis en voldoende inzicht over wat kanker in medische en psychische zin met zich meebrengt. In dit artikel komen de volgende onderwerpen aan de orde: het ontstaan van en de status quo van psycho-oncologie, de aard van psychische problemen in de context van kanker, de ontwikkeling en uitkomsten van interventiestudies en de beschikbaarheid van diverse interventiemogelijkheden in Nederland.

De prevalentie was in 2011 totaal 600.000 personen (270.000 man, 330.000 vrouw). De grootste groep betreft hierbij vrouwen met borstkanker (ongeveer 30% van de gevallen), gevolgd door mannen met prostaatkanker (ruim 20% van de gevallen), mensen met huidkanker (ruim 14%), darmkanker (rond 13%) en longkanker (vrouwen ruim 9%, mannen ruim 13%; www.​ikcnet.​nl ). De kans op kanker stijgt met de jaren: 70% is ouder dan 60 jaar, 9% is jonger dan 45 jaar en 0,6% betreft kinderen jonger dan 15 jaar. Bij kinderen is (acute) leukemie de meest gediagnosticeerde vorm van kanker, bij adolescenten en jongvolwassenen komen lymfklierkanker, zaadbalkanker en melanoom het meest voor. Borstkanker komt het meest voor bij vrouwen boven de 30 jaar Bij mannen tussen 30 en 45 jaar komt melanoom het meeste voor en prostaatkanker komt het meest voor bij mannen boven de 45 jaar. De prevalentie van kanker stijgt tegenwoordig met ongeveer 2-3 procent per jaar, onder andere door verbetering van behandelmogelijkheden en bijbehorende overlevingskansen, in combinatie met de vergrijzing. Tegenwoordig geneest ongeveer de helft van de patiënten; de overlevingskansen zijn de afgelopen decennia toegenomen, onder andere doordat kanker vaker in een vroeger stadium wordt ontdekt en moderne behandelingen effectiever zijn (bron: www.​ikcnet.​nl, www.​cijfersoverkanke​r.​nl).
