Samenvatting

Dit hoofdstuk gaat over een spontaan ontstane (primaire) artrose van het linkerheupgewricht. Opmerkelijk is dat de patiënt vrijwel alleen pijn in de linkerknie ervaart. In dat opzicht is de casus te beschouwen als een valkuil voor onderzoekers. Primaire artrose van heupen, handen, knieën en wervelkolom komt veel voor. De exacte oorzaak ervan is nog steeds niet bekend. Men beschouwt een dergelijke spontane artrose als een reumatische ziekte. Het hoofdstuk beschrijft uitgebreid de casus, symptomatologie, de talloze factoren die kunnen leiden tot artrose, en de behandeling.

Bij de decline squat test maakt de patiënt een kniebuiging terwijl de hielen zich op een verhoging bevinden. Men kan ook gebruikmaken van een speciaal schuin (decline squat) plankje. De m. quadriceps contraheert krachtig tijdens deze test. Deze pijnprovocatietest wordt toegepast voor de anterieure structuren van de knie, inclusief de patella.
