Posttraumatische anterieure schouderinstabiliteit bij sporters

Samenvatting

Een traumatische anterieure schouderluxatie is een frequent voorkomend letsel bij jonge sporters en vormt een belangrijke aanleiding tot het ontstaan van schouderinstabiliteit. In de regel vormt een eerste luxatie geen reden voor chirurgische interventies. Er is echter een tendens om jonge sporters sneller operatief te behandelen, aangezien conservatieve behandeling van deze groep tot een hoog percentage recidieven leidt. Het letsel dat frequent wordt geconstateerd en samenhangt met anterieure instabiliteit, is labrumletsel in combinatie met capsuloligamentair letsel. De chirurgische behandeling is erop gericht dit letsel te reconstrueren. Dit gebeurt de laatste decennia voornamelijk artroscopisch, hoewel een open (wekedelen)techniek nog altijd het laagste recidiefpercentage kent. Voor patiënten met botverlies als gevolg van persisterende schouderinstabiliteit bestaat allerminst consensus in chirurgische behandeling. De kans dat een artroscopische (wekedelen)behandeling echter een teleurstellend resultaat oplevert, is hoog.
