Samenvatting

De beste behandeling van PONV is de rigoureuze profylaxe. Er is een grote hoeveelheid aan publicaties beschikbaar die de effectiviteit en veiligheid van de farmacologische en niet-farmacologische opties aantonen. Om de incidentie van PONV op de PACU zoveel mogelijk te reduceren, is het nodig om een duidelijk afdelingsbeleid te implementeren en PONV profylaxe als integraal onderdeel te verankeren in de anesthesiologische praktijkvoering. Het voorkomen van PONV begint met het identificeren van de risico’s. De tweede stap is de consequente profylaxe aan de hand van het individueel risicoprofiel. Voor patiënten met een of meer risicofactoren wordt een profylaxe met anti-emetica uit verschillende groepen aanbevolen. Dezelfde principes gelden ook voor de behandeling van PONV. De voor de profylaxe van PONV gebruikelijke middelen kunnen in het geval van een onsuccesvolle profylaxe voor de behandeling van misselijkheid en braken worden ingezet. Bij een gefaalde profylaxe wordt aanbevolen om een geneesmiddel te gebruiken dat niet eerder al profylactisch werd ingezet.
