Samenvatting

Pijnlijke mictie is een veelvoorkomende klacht. De kans op een urineweginfectie hierbij is groot. Bij vrouwen is de meest voorkomende oorzaak een cystitis, bij mannen een urethritis, cystitis of prostatitis. Koorts en flankpijn kunnen op een hoge urineweginfectie (pyelonefritis) wijzen of op een acute prostatitis bij de man. Bij recidiverende urineweginfecties is het zinvol te vragen naar risicofactoren. Bij de man met urethritisklachten is het van belang aandacht te besteden aan de mogelijkheid van een geslachtsziekte en te vragen of daarmee gepaard gaande verschijnselen aanwezig zijn. Lichamelijk onderzoek is geïndiceerd bij frequent recidiverende urineweginfecties en bij patiënten die een verhoogd risico hebben op een gecompliceerd beloop. Dit betreft vrijwel altijd onderzoek van de urine. Bij verdenking op een urineweginfectie is een nitriet- en leukotest, eventueel in combinatie met een dipslide of sediment geïndiceerd. PCR-onderzoek naar een soa is aangewezen bij verdenking daarop. In specifieke gevallen is soms bloedonderzoek (o.a. nierfunctie) wenselijk, met name bij obstructieve klachten in combinatie met urineweginfecties. Radiodiagnostiek kan een waardevolle aanvulling zijn bij verdenking op bijvoorbeeld urinesteenlijden, obstructie van de urinewegen of congenitale afwijkingen.
