Perspectieven van mantelzorgende familieleden op de taal over gedragsverandering bij dementie: een online ‘mixed-methods’-onderzoek

Samenvatting

Doelstelling: Sinds de Twittercampagne #BanBPSD is er veel aandacht voor de terminologie die wordt gebruikt voor ‘gedragsveranderingen’ bij dementie. Maar in de opiniestukken en de eerste studies hierover is het perspectief van mantelzorgende familieleden enigszins onderbelicht gebleven. Dit onderzoek bekijkt de standpunten van mantelzorgers over de terminologie en het taalgebruik voor dit kader.
Methode: een ‘mixed-methods’ online vragenlijst, afgenomen bij mantelzorgende familieleden. De voorkeuren voor taalgebruik werden onderzocht en nader bekeken met behulp van ondersteunende open vragen over de redenen voor de gemaakte keuzes.
Resultaten: Ongeveer 229 mantelzorgende familieleden vulden de vragenlijst in. Termen waar de respondenten het meest een hekel aan hadden waren ‘lastig gedrag’, ‘gedrag dat lastig is om mee om te gaan’ en ‘gedrags- en psychologische symptomen van dementie’. De meest geprefereerde term was een nieuw begrip: ‘gedrags- en emotionele uitingen van een behoefte’, een term die weinig mensen al eerder hadden gehoord. Over het algemeen hadden mantelzorgers een voorkeur voor positief geformuleerde, gemakkelijk begrijpelijke, persoonsgerichte termen die gedragsveranderingen toeschreven aan onvervulde behoeften, waarmee de rol van de mantelzorger in het managen ervan ook werd erkend.
Conclusies: Aangezien mantelzorgers vaak degenen zijn die veranderingen uitvoeren en er ook vaak beroep op hen wordt gedaan om namens de patiënt beslissingen te nemen, is het belangrijk dat professionals de tijd nemen om na te gaan hoe mantelzorgers terminologie begrijpen, en om bij het aanbieden van gerichte interventies voldoende aandacht te schenken aan hun taalgebruik. Deze bevindingen lijken uit te wijzen dat het raadzaam is om termen die vaak worden gebruikt voor gedragsveranderingen bij dementie te vermijden, zoals lastig gedrag (CB: ‘challenging behaviour’, red.), ‘gedrag dat lastig is om mee om te gaan’ (BtC: ‘behaviour that challenges’, red.) en BPSD (‘behavioural and psychological symptoms of dementia’ of ‘gedrags- en psychologische symptomen van dementie’, red.).
