23 Percutane coronaire interventies

Samenvatting

Myocardrevascularisatie blijft de belangrijkste doelstelling bij de behandeling van patiënten met obstructief coronairlijden, omdat duidelijk is aangetoond dat deze behandelingsvorm, meer nog dan de medicamenteuze behandeling, behalve de symptomen, in bepaalde groepen ook de prognose verbetert. In tegenstelling tot de klassieke coronaire bypassoperatie (CABG) beoogt men met een percutane coronaire interventie (PCI) langs niet-invasieve weg rechtstreeks de coronaire vernauwingen op te heffen. De techniek werd in 1977 geïntroduceerd door Gruentzig in Zürich. Tien jaar later werd op een controleangiogram aangetoond dat de gedilateerde coronaire arterie mooi open was. Geen enkele therapeutische interventie in de cardiologie heeft in zo korte tijd een zo snelle evolutie gekend als PCI. In de meeste westerse landen overtreft het aantal PCI’s dat per jaar wordt uitgevoerd het aantal CABG’s, en dit verschil neemt nog steeds toe.
