14. Patiëntgerichte strategieën

Samenvatting

In toenemende mate wordt ook de patiënt ingeschakeld bij het op gang brengen van verbeteringen in de zorg. Dit kan aan de orde zijn in alle fasen van de gang van de patiënt door de zorg. Door het geven van informatie over gezondheidsproblemen, zelfzorg en professionele zorg aan mensen die nog geen ‘patiënt’ zijn, kan men proberen het ziektegedrag te beïnvloeden en zo (nieuwe of bestaande) inzichten in de praktijk brengen. Publieke informatie over kwaliteit, toegang, kosten en uitkomsten van zorg kan de patiënt helpen bij het maken van een keuze tussen verschillende zorgaanbieders. Dit past in een filosofie van marktwerking in de gezondheidszorg. Er is bewijs dat informatie over de kwaliteit van zorg (nog?) weinig invloed heeft op het keuzegedrag van consumenten voor ziekenhuizen en individuele artsen. Het betrekken van de patiënt bij de zorgverlening kan implementatie van nieuwe werkwijzen eveneens ondersteunen. Ter voorbereiding van contacten met zorgverleners kan een patiënt zijn gezondheidsproblemen, kwaliteit van leven en behoeften aan zorg schriftelijk vastleggen en inbrengen in het contact. Effecten op gezondheidsuitkomsten of zorgconsumptie hiervan zijn niet aangetoond. Een patiëntgerichte benadering en goede informatieverstrekking tijdens contacten lijken samen te hangen met een betere therapietrouw van de patiënt ten aanzien van behandeling en adviezen. Keuzehulpen, ofwel decision aids, zijn gestructureerde patiënteneducatiematerialen die bedoeld zijn om patiënten te stimuleren een actieve rol in te nemen in hun zorg door hen te ondersteunen bij het nemen van een beslissing. Keuzehulpen hebben positieve effecten op kennis, realistische verwachtingen, twijfel aan de beslissing en actieve betrokkenheid in de besluitvorming, maar geen effect op tevredenheid met de besluitvorming, angst en gezondheidsuitkomsten.
