Samenvatting

In dit hoofdstuk worden ontstaan, kenmerken, diagnostiek en behandeling van parafilieën besproken. Hoewel enige parafiele interesse veel voorkomt, is er maar een kleine groep mensen die zich veel of uitsluitend met parafiele seksualiteit bezighoudt. Een parafilie is echter pas een stoornis als iemand er zelf onder lijdt of handelt met een niet-instemmende ander. Behandeling van de parafiele interesse op zich heeft daarom in principe geen prioriteit. Het is echter wel mogelijk en zeker relevant als er sprake is van schade of gevaar voor de patiënt zelf of anderen. Openheid en oprechte interesse met betrekking tot de parafiele seksuele beleving van de patiënt zijn belangrijke kernwaarden voor de behandelaar van parafiele stoornissen. Het is de uitdaging om gezamenlijk met de patiënt (en eventueel de partner) uit te vinden hoe de seksualiteit een bevredigende vorm kan krijgen die geen schade of gevaar oplevert voor zichzelf of anderen.

Dit is een deductie uit verschillende bestaande onderzoeken; er zijn hiervoor geen specifieke prevalentiegegevens uit onderzoek beschikbaar. Prevalentie pedofilie ≈ 0,3 %; prevalentie seksueel delictgedrag ≈ 0,74 %, waarvan ongeveer de helft kindermisbruik betreft: prevalentie kindermisbruik ≈ 0,37 %. Van die misbruikplegers is een kwart tot de helft pedofiel: prevalentie pedofiele plegers ≈ 0,12 tot 0,18 %. Prevalentie pedofilie ≈ 0,3 %, prevalentie pedofiele plegers ≈ 0,12 tot 0,18 %, dus ≈ 40 tot 60 % van de pedofiele mannen zou pleger kunnen zijn [52].
