Samenvatting

Dit hoofdstuk over het pancreas begint met een inleiding over embryologie, anatomie, fysiologie en beeldvormende diagnostiek (transabdominale echografie, CT, MRI, PET, endo-echografie, endoscopische retrograde cholangiopancreaticografie (ERCP) en diagnostische laparoscopie). Daarna worden de drie meest voorkomende afwijkingen besproken: acute pancreatitis, chronische pancreatitis (met name de behandeling van pijnklachten) en pancreastumoren. Veel aandacht gaat uit naar de verschillende pancreastumoren: pancreas ductaal adenocarcinoom (PDAC) van pancreaskop, het corpus en van de staart, periampullaire carcinomen zoals papil van Vater carcinoom, distaal cholangiocarcinoom en duodenumcarcinoom. Daarnaast komen intraductale papillaire mucineuze neoplasma (IPMN), de neuro-endocriene tumoren en overige tumoren aan bod. De beschikbare behandelingen worden voor tumoren in de pancreaskopregio belicht, om te beginnen de drie varianten van pancreatoduodenectomie: pylorussparende pancreatoduodenectomie (PPPD), pylorusresectie pancreatoduodenectomie (PRPD) en de klassieke Whipple-operatie.
