20. Overslaan van het hart (extrasystole)

Samenvatting beleid

Indien iemand komt met de klacht van overslaan of bonken van het hart is het zinvol anamnestisch na te gaan of er mogelijk een hartaandoening aanwezig is. Ook zeer frequente extrasystolen met hemodynamische consequenties moeten worden uitgesloten. Bij lichamelijk onderzoek zal meestal geen extrasystole aanwezig zijn, objectivering met een 24-uurs-ECG is bij mensen zonder risicofactoren niet noodzakelijk. Indien een extrasystole wordt gevoeld, is objectivering ervan met behulp van een ECG zinvol, ook om een andere ritmestoornis uit te sluiten. De behandeling berust primair op voorlichting en geruststelling, daarnaast wordt aandacht geschonken aan factoren die extrasystolen kunnen uitlokken (sympathicomimetica, alcohol, nicotine, drugs). Indien de klachten erg hinderlijk zijn, kunnen eventueel bètablokkers of calciumantagonisten worden geprobeerd. Blijven de klachten ondanks de therapie gedurende langere tijd hinderlijk aanwezig, dan is het zinvol om de patiënt naar de cardioloog te verwijzen voor verder onderzoek, zeker indien multipele kamerextrasystolen aanwezig zijn.
