Overdrachten in de palliatieve zorg aan de huisartsenpost

Samenvatting

Doel

De zorg buiten kantooruren is in Nederland gereorganiseerd, net als in Engeland en Denemarken. Huisartsen werken nu vanuit grootschalige huisartsenposten. Patiënten reageren in het algemeen positief, maar spreken soms ook hun bezorgdheid uit over de kwaliteit van zorg die patiënten in de palliatieve fase tijdens de diensten krijgen. In de palliatieve zorg is continuïteit van zorg van groot belang. Om die te garanderen is een adequate informatieoverdracht nodig. In dit onderzoek zijn we nagegaan hoe het op de huisartsenpost staat met de beschikbaarheid van informatie over patiënten in palliatieve zorg, wat de inhoud van die informatie is en welk effect informatieoverdracht over deze patiëntengroep heeft.

Methode

We hebben een exploratief onderzoek gedaan naar alle telefoontjes met betrekking tot palliatieve zorg die huisartsen gedurende een jaar met de centrale huisartsenposten van Amsterdam hebben gevoerd.

Resultaten

Het totale aantal telefoontjes over patiënten die palliatieve zorg nodig hadden betrof 0,75% van alle telefoontjes naar de huisartsenpost. De huisartsen droegen informatie over in 25% van deze telefoontjes, maar als het een patiënt uit een verzorgingshuis betrof ging het om 12%. Bij patiënten die terminaal ziek waren steeg het aantal informatieoverdrachten naar 29%. Meestal betrof de informatie medische gegevens. In meer dan 90% van de gevallen ging het om informatie over de diagnose en huidige problemen, 45% betrof informatie over de wensen van de patiënt en 30% ging over de psychosociale situatie van de patiënt. De waarnemer legde een visite af na 53% van de telefoontjes over patiënten in palliatieve zorg. Als huisartsen informatie hadden overgedragen, kregen minder patiënten een verwijzing naar het ziekenhuis.

Conclusie

Huisartsen schrijven in veel gevallen geen overdracht over hun patiënten in palliatieve zorg ten behoeve van de huisartsenpost. Waarnemers die op de post werken moeten dus vaak palliatieve zorg verlenen in complexe situaties zonder dat ze adequate informatie tot hun beschikking hebben. Het is belangrijk om huisartsen aan te moedigen en te trainen om deze informatie voor de huisartsenposten beschikbaar te maken.
