15. Organisatorische implementatiestrategieën

Samenvatting

Geplande verandering in een of meer aspecten van de organisatie van de patiëntenzorg kan een belangrijke bijdrage leveren aan het succes van implementatie. Het overnemen van taken van artsen door verpleegkundigen en andere niet-artsen in de zorg (aanpassing van professionele rollen) leidt meestal tot kwaliteit van minimaal hetzelfde niveau, terwijl de effecten op de kosten van zorg gemengd zijn. Versterking van patiëntenzorgteams door middel van training, verbeterde coördinatie of uitbreiding van expertise kan positieve effecten kan hebben op zorgverlening en mogelijk ook op doelmatigheid. Professioneel handelen en patiëntuitkomsten kunnen worden verbeterd door beter gebruik van computers in de patiëntenzorg, vooral voor patiëntdossiers, herinneringen en beslissingsondersteuning. Geïntegreerde zorg (waaronder ketenzorg en zorgpaden) heeft duidelijk positieve effecten op de kwaliteit en de kosten van zorg. De interventies die in de zorgprogramma’s worden gehanteerd, verschillen onderling sterk. De effectiviteit van kwaliteitsmanagement op professioneel handelen en patiëntuitkomsten is niet overtuigend aangetoond. De invloed van cultuur en leiderschap op professioneel handelen en uitkomsten lijkt plausibel, maar is evenmin goed aangetoond.
