15. Oorsuizen (tinnitus)

Samenvatting beleid

Als iemand komt met de klacht oorsuizen, sluit dan eerst behandelbare/ernstige oorzaken uit door anamnese en inspectie van de gehoorgang; doe bij twijfel een gehoortest. Bij gehoorverlies zonder duidelijke uitwendige en middenoorproblemen en vooral bij unilaterale tinnitus is nader onderzoek wenselijk om processen uit te sluiten. Eventuele medicatie moet nagekeken worden op ototoxiciteit. Bij een pulserend geluid kan naar een mogelijk operabele vaatabnormaliteit gezocht worden. Als er geen oorzaak wordt gevonden, geef dan uitleg: het komt zeer veel voor, het kan geen kwaad en het gaat vaak weer over of vermindert door gewenning. Toon empathie dat het oorsuizen zeer hinderlijk kan zijn. Van medicijnen hoeft de patiënt niet veel te verwachten. Slaapmedicatie, antidepressiva of anxiolytica zijn alleen zinvol als er duidelijke aanwijzingen zijn voor psychisch lijden en andere therapieën geprobeerd zijn. Als de klachten erg hinderlijk zijn is verwijzing naar de tweede lijn of een gespecialiseerd centrum zinvol. Hier kan gekozen worden voor medicatie, een vorm van psychotherapie, of toepassing van apparatuur. Raad aan de informatie na te lezen op www.​thuisarts.​nl/​oorsuizen/​ik-heb-last-van-oorsuizen.
