Omgaan met hoop in de palliatieve zorg

Hoop is belangrijk in de palliatieve fase. Hoop kan huisartsen voor een lastig dilemma plaatsen, bijvoorbeeld wanneer de patiënt valse, dat wil zeggen niet-realistische hoop koestert. Patiënten en zorgverleners benaderen het thema ‘hoop’ echter niet uitsluitend vanuit een realistisch perspectief, maar ook vanuit een functioneel en een narratief perspectief: om de situatie aan te kunnen en omdat het past bij een levensverhaal. Als de huisarts deze perspectieven alledrie in het oog weet te houden, komt dat de communicatie in de laatste levensfase ten goede. De balans tussen hoop, hopeloosheid en wanhoop – de dynamiek van hoop – hangt samen met veranderingen in de fysieke conditie. Huisartsen kunnen zulke veranderingen in de palliatieve fase gebruiken als aangrijpingspunt om het thema ‘hoop’ aan de orde te stellen. Tot slot moeten huisartsen vanuit de relationele ethiek van hoop een balans zien te vinden tussen het aanspreken van hoopgevende elementen (empowerment) enerzijds en de erkenning van verlies en lijden (compassie) anderzijds. Een goede relationele ethiek van hoop vraagt verder dat de huisarts ook zijn eigen hoop en lijden onder ogen ziet.
