Nutritionele screening bij de oncogeriatrische patiënt

Samenvatting

Een patient wordt niet als geriatrisch beoordeeld op basis van zijn leeftijd maar op basis van zijn functioneel profiel. Veroudering gaat gepaard met wijzigingen in verschillende organen en een verlies van functionele reservecapaciteit, wat kan leiden tot een verhoogde kwetsbaarheid. Bij een oudere patient met kanker kunnen er problemen optreden met de voedingsinname naast de gevolgen van het verouderingsproces zelf en de inflammatoire effecten die uitgelokt worden door de tumor. Wanneer deze veranderingen vertaald worden naar nutritionele concepten dan wordt het duidelijk dat er bij de oudere kankerpatiënt een sterke overlapping is van vasten, sarcopenie en cachexie.
Het bepalen van de voedingsstatus zou deel moeten uitmaken van het standaardonderzoek van de oudere kankerpatiënt. Er moet een onderscheid gemaakt worden tussen het opsporen van een ondervoedingsrisico en het bepalen van de reële voedingsstatus met de daartoe geëigende parameters. Bepaling van het lichaamsgewicht met aandacht voor recent gewichtsverlies zijn hierin essentieel. Ook de klassieke BMI (Body Mass Index) dient met voorzichtigheid geïnterpreteerd te worden. Hoewel de BMI systematisch overschat wordt bij ouderen, houdt een lage BMI toch een risico in voor ondervoeding. Deze verhoogde aandacht voor latente voedingsproblemen kan vroege interventies mogelijk maken en zou ingebed moeten worden in een bredere geriatrische evaluatie.
