Inleiding

Het non-hodgkinlymfoom (NHL) is een verzamelnaam voor ongeveer vijftig verschillende klinisch-pathologische entiteiten die zijn omschreven in de WHO-classificatie van 2008.1 NHL is een zeldzame ziekte, met in westerse landen een incidentie van 10-15 per 100.000 in de algemene bevolking. De huisarts zal elke 3 jaar gemiddeld één nieuwe patiënt tegenkomen. De incidentie neemt toe met de leeftijd: 60% van de patiënten is ten tijde van de diagnose ouder dan 60 jaar. In 90% van de gevallen ontstaat NHL in een ontspoorde B-lymfocyt, slechts 10% ontstaat in een T-cel. In dit hoofdstuk bespreken wij de twee vaakst voorkomende subtypes: het folliculair non-hodgkinlymfoom (FL ), dat tamelijk indolent is en 20-30% van alle NHL uitmaakt, en het diffuus grootcellig B-cellymfoom (DLBCL ), dat agressief is en 40% van alle NHL uitmaakt.
