Somatisch onvoldoende verklaarde klachten (SOLK) zijn vaak complex. Soms aarzelt de arts om eventuele psychische factoren ter sprake te brengen, omdat hij denkt dat de patiënt daar weinig inzicht in heeft. Als het gaat om patiënten van niet-westerse afkomst, kan de situatie nog complexer worden door communicatieproblemen en verschillen in opvattingen over ziekte. Veel huisartsen nemen aan dat er in niet-westerse culturen een stigma rust op psychische klachten, maar dit stigma is minder sterk dan men denkt. Als niet-westerse allochtonen een klacht als somatisch presenteren, betekent dat niet per se dat zij de psychische oorzaken ontkennen: in veel culturen is de scheiding van lichaam en geest niet zo sterk als in de westerse cultuur. Het is dan ook zeker mogelijk, en zelfs aan te bevelen, om psychische factoren ter sprake te brengen in het consult. De NHG-Standaard SOLK bevat bruikbare adviezen en geeft een model om de anamnese te structureren: SCEGS (somatisch-cognitief-emotioneel-gedragsmatig-sociaal). Bij niet-westerse allochtonen is het beter de volgorde te veranderen naar SGSCE, waardoor de gedragsmatige en sociale consequenties van de klacht voorop komen te staan. Het is belangrijk een vertrouwensrelatie op te bouwen met de patiënt, zo nodig met behulp van het cultureel interview. Bij het aan de orde stellen van psychische klachten kan het helpen te beginnen bij de basale emoties (boos-bang-blij-bedroefd), metaforen te gebruiken en waar mogelijk het probleem te omschrijven in de eigen woorden van de patiënt. Van belang is ook het beroepsgeheim te benadrukken.
