Niet-suïcidale zelfbeschadiging/Nonsuicidal Self-Injury (NSSI)

Niet-suïcidale zelfbeschadiging verwijst naar het doelbewust aanbrengen van fysieke schade aan het eigen lichaam zonder de intentie om zichzelf het leven te benemen, zoals zichzelf krassen, snijden en branden (Claes en Vandereycken 2007a). Niet-suïcidale zelfbeschadiging is een ernstig probleem dat vele jongeren treft en dat ernstige gevolgen kan hebben. Onderzoek toont aan dat meer dan 70 % van de jongeren met een geschiedenis van niet-suïcidale zelfbeschadiging ooit een suïcidepoging onderneemt (Nock et al. 2006). De frequentie van de zelfbeschadiging, de ernst van de wonden en het aantal verschillende methoden dat gehanteerd wordt om zichzelf te beschadigen, worden beschouwd als aanwijzingen voor de ernst van niet-suïcidale zelfbeschadiging. Gegeven de ernst van niet-suïcidale zelfbeschadiging zien we een toenemende interesse voor dit gedrag in de maatschappij en worden jongeren – via sociale media – geïnformeerd over dit gedrag, de gevolgen en de aanpak ervan. In het Nederlandse taalgebied kan niet-suïcidale zelfbeschadiging geïnventariseerd worden met behulp van de Zelfverwondingsvragenlijst (SIQ-TR; Claes en Vandereycken 2007b) of de Nederlandse versie van de Brief Non-Suicidal Self-Injury Assessment Tool (BNSSI-AT-NL; Baetens en Claes 2011). In de DSM-IV-TR (APA 2000) kan niet-suïcidale zelfbeschadiging enkel gediagnosticeerd worden als kenmerk van de borderlinepersoonlijkheidsstoornis. Verschillende studies hebben echter aangetoond dat zelfbeschadiging zowel bij jongeren als (jong)volwassenen kan voorkomen zonder dat er sprake moet zijn van een borderlinepersoonlijkheidsstoornis (Glenn en Klonsky 2013; Selby et al. 2012) of een andere psychische stoornis. Daarom werd er reeds gedurende decennia door verschillende onderzoekers gepleit om zelfbeschadiging als een apart categorie te introduceren in de DSM (Favazza en Rosenthal 1990; Kahan en Pattison 1984; Muehlenkamp 2005), en slaagden Shaffran en Jacobson (2009) erin om het NSSI-syndroom te introduceren in de DSM-5 (APA 2013, 2014) als aandoening die verder onderzoek behoeft. …
