Mondzorg bij diabetescontrole?

Tussen parodontitis en diabetes mellitus bestaat een wisselwerking. De chronische hyperglykemie veroorzaakt een inflammatie van het parodontium die (veelal irreversibele) schade toebrengt aan het steunweefsel van de gebitselementen, zodat er uiteindelijk tanden verloren gaan. De parodontitis op zijn beurt zorgt voor een systemische stijging van cytokines zoals de interleukines 1 en 6, en tumornecrosefactor-alfa. Deze ontstekingsmediatoren bevorderen de insulineresistentie en daarmee de kans op (verergering van) diabetes. Verscheidene onderzoeken hebben laten zien dat het HbA1C lager wordt – en dus de glucoseregulatie verbetert – wanneer men de paradontitis behandelt.

Omdat deze wisselwerking niet erg bekend is bij patiënten en evenmin veel aandacht krijgt van behandelaars, pleiten wij ervoor om bij de diabetescontrole extra aandacht te besteden aan de gebitsstatus. Het zou zinvol zijn als alle bij de diabeteszorg betrokken behandelaars – onder wie ook tandartsen en mondhygiënisten – onderling afspraken maken over hoe te handelen bij symptomen van parodontitis.
