Mogelijke additionele voordelen van preconceptiezorg bij de vrouw voor de volksgezondheid

Een literatuurstudie

Samenvatting

In deze literatuurstudie wordt verslag gedaan van de internationale wetenschappelijke evidentie van de mogelijke positieve invloed van preconceptiezorg op de gezondheid van vrouwen en daarmee op de zwangerschap, de groei en ontwikkeling van het ongeboren kind en de gezondheid na de geboorte. Als gezondheidsthema heeft de reproductieve gezondheid een belangrijke plek op de agenda van de volksgezondheid verworven. Er is steeds meer wetenschappelijk bewijs voor het belang van preconceptiezorg binnen de reproductieve gezondheidszorg, maar ook voor het belang van de gezondheid van de algemene bevolking. In deze literatuurstudie richten we ons voornamelijk op gezondheidsverbeteringen die worden bereikt via preconceptiezorg die tevens positieve, mogelijk blijvende, effecten hebben op de publieke gezondheid. Van negen domeinen en daarbinnen van specifieke risico’s binnen de preconceptionele fase, zijn de maternale gezondheidsvoordelen beschreven als gevolg van effectieve preconceptiezorg. De resultaten laten zien dat met passende maatregelen duidelijke gezondheidsvoordelen te behalen zijn; bij de preventie en aanpak van fysiek- en seksueel geweld, roken, het gebruik van opiaten, alcoholmisbruik, vitamine B12 deficiëntie, over- en ondergewicht, voedingsdeficiënties, infectieziekten zoals tuberculose, HIV, hepatitis B en C, malaria en syfilis, en blootstelling aan schadelijke milieufactoren. Preconceptiezorg biedt een ‘window of opportunity’ voor zowel de verbetering van de reproductieve gezondheid als de algemene volksgezondheid.
