Samenvatting

Astma werd voorheen gezien als een aandoening die louter de grote luchtwegen betrof. Op zich is het niet verwonderlijk dat de aandacht voor de kleine luchtwegen achterbleef: astma is een aandoening waarbij de eensecondewaarde (FEV1), als maat voor met name centrale luchtwegobstructie, verlaagd is. Na toediening van luchtwegverwijders is er herstel van deze obstructie, waarbij ook de klachten verminderen. Pas met de toegenomen diagnostische mogelijkheden om perifere obstructie (in de luchtwegen met een diameter minder dan 2 mm) te bepalen en te behandelen, is er meer aandacht voor de bijdrage van kleine luchtwegen in astma.
