Transcranial direct current stimulation als behandeling voor auditieve hallucinaties

Samenvatting

Auditieve hallucinaties zijn een veelvoorkomend symptoom van schizofrenie. Ook bij andere diagnosen, zoals borderline persoonlijkheidsstoornis, angststoornissen, affectieve stoornissen, posttraumatische stressstoornis en autisme komen stemmen regelmatig voor. Vaak reageren stemmen goed op antipsychotica en/of cognitieve gedragstherapie. Er is echter een significante minderheid van zo’n 25–30 % van de patiënten bij wie deze hallucinaties blijven voortbestaan, ondanks adequate behandeling. Alternatieve behandelingen voor deze therapieresistente groep patiënten zijn schaars en meestal enkel gericht op het leren omgaan met de hallucinaties. Het vinden van een effectieve behandeling die auditieve hallucinaties in deze groep kan doen verminderen, is daarom van groot belang. Transcranial direct current stimulation (tDCS) is een veilige en non-invasieve techniek die de corticale prikkelbaarheid kan beïnvloeden door de toepassing van zeer lage elektrische gelijkstroom. Deze techniek wordt momenteel veel onderzocht als behandelingsmogelijkheid voor neurologische en psychiatrische symptomen. Er zijn positieve resultaten gevonden voor symptomen van depressie, bipolaire stoornis, schizofrenie, de ziekte van Alzheimer, de ziekte van Parkinson, epilepsie en beroerte, maar de toepassing ervan voor auditieve hallucinaties is nog relatief nieuw. Dit artikel biedt een overzicht van de huidige stand van zaken in dit veld en bespreekt richtlijnen voor toekomstig onderzoek.
