Abstract

Glanzend witte gebitselementen zijn meer dan ooit een visitekaartje. Met een mooie glimlach is het zoveel gemakkelijker je zelfverzekerd te voelen. Amerikaanse ‘celebs’ en ‘wannabees’ weten het al langer. Zij spenderen fortuinen aan de ‘Hollywood smile’. Bij ons loopt het zo’n vaart nog niet, hoewel steeds meer tandartsen de vraag krijgen om gebitselementen witter te maken. Ook reclames hebben een sturende invloed op de wensen van de patiënt. Het lijkt er daarom op dat we bij de vraag om te bleken meer met consumenten dan met patiënten te maken hebben. ‘Tandarts, kunt u mijn tanden bleken?’ is voor de patiënt een eenvoudige vraag. Voor de tandarts gaat het om een ethische vraag. Het principe ‘primum non nocere’ of ‘geen schade aanrichten’ dient vooropgesteld te worden. Feit is dat als bleken geen kwaad doet, het witter maken van de gebitselementen dan onder het principe ‘weldoen’ valt en dus ethisch geoorloofd is. Als tandarts moet men zich autonoom opstellen en over de nodige kennis beschikken om een correcte diagnose te stellen en het meest ideale behandelplan voor te leggen. Men dient zich te realiseren dat als psychische problemen ten grondslag liggen aan het verzoek om te bleken, deze niet worden opgelost door het bleken. Soms voelt men in het eerste consult al aan dat het om een waanachtig idee gaat, een obsessie, een verouderingsproces dat als traumatisch wordt ervaren. Volgens de patiënt is er iets mis, maar volgens anderen niet. Men spreekt van een gestoorde lichaamsbeleving of ‘body dismorphic disorder’. Toch lijkt het erop dat men zijn patiënt tekortdoet door al te rigoureus te weigeren zijn gebitselementen witter te maken. Een weigering om het probleem van de patiënt te behandelen drijft deze mensen naar bleekklinieken, bemand door niet-tandartsen of naar al dan niet effectieve en soms zelfs schadelijke middelen en methoden die via internet te verkrijgen zijn. Het is de taak van de tandarts de patiënt te begeleiden op zijn zoektocht naar een middel om zijn dentitie te bleken en ervoor te pleiten dat alleen een tandarts zou mogen bleken.
