Mentaliseren, pseudomentaliseren en dismentaliseren

Het belang van een uitgebreide op maat gesneden beginfase voor het aangaan van een therapeutische relatie met forensisch-psychiatrische patiënten

Samenvatting

‘Mentalization-Based Treatment’ ofwel mentaliserenbevorderende therapie wordt binnen de forensische psychiatrie in Nederland en daarbuiten steeds meer toegepast. Dit heeft alles te maken met de grote aandacht voor de interactie tussen behandelaar en patiënt en de therapeutische relatie, rekening houdend met vroege ontwikkelingsstoornissen. Deze zijn ontstaan door problemen met zich veilig hechten, onder andere door vroeg opgelopen psychotrauma’s. Dit vraagt aan het begin van de behandeling nadrukkelijk om een voorzichtige en rustige contactname met deze patiënten, terwijl zij juist met hun gedrag uitnodigen tot confrontatie en correctie. In dit artikel wordt ervoor gepleit om bij aanvang van de behandeling meer impliciet en indirect een op de primaire relatie gericht contact aan te gaan, waarbij vaktherapieën een belangrijke rol spelen. Zo kan langzamerhand een vertrouwensband ontstaan, waarop de methodiek van leren mentaliseren kan worden gefundeerd. Als men te vroeg met dergelijke methodische behandelingen begint, kunnen patiënten en therapeuten gaan pseudomentaliseren en dismentaliseren.
