26. Menstruatiepijn (primaire dysmenorroe)

Samenvatting beleid

Primaire dysmenorroe is een tijdens de menstruaties optredende krampende pijn in de onderbuik zonder aanwijzingen voor een ziekte als verklaring. Diagnostisch onderzoek is gericht op het uitsluiten van pathologische oorzaken van menstruatiepijn (secundaire dysmenorroe). Hiertoe volstaan meestal voorgeschiedenis, anamnese en – meestal niet nodig vlak na de menarche – speculumonderzoek en vaginaal toucher. Begin de behandeling met uitleg en geruststelling onder verwijzing naar www.​thuisarts.​nl/​vaginaal-bloedverlies/​ik-heb-menstruatiepijn. Adviezen: warmte in de vorm van een warmwaterzak of kruik is effectief, lichamelijke activiteit in mindere mate. Bij langdurige klachten of contra-indicaties voor medicamenteuze behandeling kan toepassing van elektrostimulatie in de vorm van TENS overwogen worden. Cognitief-gedragsmatige behandeling en ontspanningsoefeningen zijn bij sommige vrouwen mogelijk werkzaam. Medicamenteuze behandeling: NSAID’s leiden tot substantiële verbetering en moeten in of vlak voor het begin van de menstruatie ingenomen worden, bij maagklachten samen met een protonpompremmer, zo nodig wisselen van preparaat. Bij onvoldoende effect, contra-indicaties voor NSAID’s of gelijktijdige anticonceptiewens ligt het voorschrijven van orale anticonceptiva (OAC’s) voor de hand. Deze zijn gemiddeld effectief. Eventueel kunnen NSAID’s en OAC’s gecombineerd worden. Bij ernstige klachten en onvoldoende effect in drie cycli of toename van de klachten moet men alsnog verwijzing naar een gynaecoloog overwegen.
