30. Mechanische beademing

Samenvatting

Dit hoofdstuk leert je alle ins en outs van mechanische beademing. Eerst gaan we in op de basisbegrippen van beademingscurves: druk, flow, volume, capnografie, saturatie en plethysmografie. Na uitleg over het functioneren van de longen, met uitleg over weerstand en compliantie, richten we het vizier op de instellingen. Begrippen als trigger, frequentie en I:E-ratio, drukstijgtijd, expiratoire trigger sensitivity, flow, en Pmax, drukbegrenzing en limiet worden behandeld. Indicaties voor PEEP (positieve eindexpiratoire druk) worden inzichtelijk gemaakt. De verschillende beademingsvormen worden ingedeeld op basis van adempatroon, targetschema, faseparameter (triggering, limiet, cycling en PEEP) of flowpatroon (volume control of pressure control). Veel toegepaste beademingsvormen worden toegelicht en ook de diverse strategieën van mechanische beademing komen aan bod: longprotectieve strategie en obstructieve strategie bij astma en COPD. Ten slotte is er informatie over ontwennen (weanen) en detuberen.
