ME/CVS, de psychotherapeut en de (on)macht van het evidencebeest

Samenvatting

Cognitieve gedragstherapie (CGT) / progressieve inspanningstherapie (GET) is volgens opiniemakers de enige bewezen effectieve behandeling voor myalgische encephalomyelitis (ME) / chronischevermoeidheidssyndroom (CVS). Onderzoek toont echter aan dat de psychosociale verklaringsmodellen van Vercoulen en anderen,de rechtvaardiging voor CGT/GET, niet houdbaar zijn. CGT/GET is niet alleen ineffectief, maar zelfs potentieel schadelijk voor een grote groep ME/CVS-patiënten. Karakteristieke symptomen, zoals pijn, vermoeidheid en cognitieve klachten, maar bovenal verergering van klachten na een minimale inspanning (post-exertional malaise) kunnen op plausibele wijze medisch verklaard worden uit intracellulaire inflammatie, oxidatieve / nitrosatieve stress en gevolgschade daarvan, metabole dysfunctie, gastro-intestinale abnormaliteiten enzovoort. De dominantie van het psychosociale verklaringsmodel heeft grote gevolgen voor patiënten: medisch, juridisch, psychisch en sociaal. Psychotherapeuten en psychologen zouden vraagtekens moeten zetten bij een rol als behandelaar van ME/CVS. Doen zij dat niet, dan zullen ME/CVS-patiënten geneigd zijn om psychologische begeleiding te mijden terwijl die wel helpend kan zijn bij het omgaan met de ziekte.
