Mate van gevoelsverlies voorspelt het risico op de diabetische voet

Samenvatting

De in 2013 gestarte Rotterdam Diabetic Foot (RDF) Study had tot doel perifere zenuwfunctie (gevoelsverlies) bij diabetespatiënten te relateren aan het risico op disbalans, vallen en het oplopen van diabetische voetulcera, de uitkomstmaten van dit onderzoek. Hiertoe werden 416 diabetespatiënten gevolgd in het Franciscus Gasthuis & Vlietland ziekenhuis in Rotterdam die aan een testbatterij van 39 items werden onderworpen. De mediane follow-up besloeg 956 dagen. Van de 416 patiënten ontwikkelden 40 patiënten een voetulcus. Onder hen waren meer mannen dan vrouwen; ze hadden vaker een lager lichaamsgewicht, vaker retinopathie en meer klachten van neuropathie dan de patiënten die geen ulcus ontwikkelden. Tevens hadden ze ernstiger gevoelsverlies aan de voeten, vaker een voorgeschiedenis van ulcera en amputaties, en minder vaak palpabele voetarteriën. Zeven patiënten hadden vaker achter elkaar meerdere episoden van ulcera. Uit de resultaten is geconcludeerd dat het graderen van gevoelsverlies een betere risicostratificatie oplevert in vergelijking met de tests uit de huidige richtlijn (10 g monofilament en/of stemvork). Dit artikel geeft een beknopte beschrijving van de studie en beschrijft hoe tot deze conclusie is gekomen.
