VALIDADE DE TESTES DE CAMPO PARA ESTIMATIVA DA APTIDÃO CARDIORRESPIRATÓRIA EM CRIANÇAS E ADOLESCENTES: UMA REVISÃO SISTEMÁTICA

RESUMO

Objetivo:

Revisar sistematicamente a literatura para verificar a validade dos testes de campo para avaliação da aptidão cardiorrespiratória (ACR) em crianças e adolescentes.

Fontes de dados:

Foram utilizadas as bases de dados: Medline (PUBMED), SPORTDiscus, Scopus, Web of Science, além das bases latino-americanas LILACS e SciELO. A busca compreendeu todo o período de existência das bases até fevereiro de 2015, em idioma inglês e português. Todas as etapas do processo foram previstas de acordo com o PRISMA.

Síntese dos dados:

Após verificação dos critérios de inclusão, elegibilidade e qualidade dos estudos, 43 trabalhos foram analisados na íntegra, sendo obtidos 38 por meio da busca nas bases de dados eletrônicas e cinco por meio de biblioteca particular e referências de outros artigos. Do total dos artigos, apenas 13 foram considerados de alta qualidade segundo os critérios adotados. O teste mais investigado na literatura foi o shuttle run de 20 metros (SR-20 m), contabilizando 23 trabalhos, seguido pelos testes de distâncias entre 550 metros e 1 milha com 9 estudos, os testes com tempos de 6, 9 e 12 minutos também com 9 estudos e, por fim, os protocolos de banco e novas propostas de teste representados por 7 trabalhos.

Conclusões:

O teste SR-20 m parece ser o mais apropriado para avaliação da ACR de jovens, com a equação de Barnett, recomendada para estimativa do VO2pico. Como segunda alternativa para avaliação da ACR, indica-se o teste de 1 milha e, a equação proposta por Cureton, para estimativa do VO2pico.

ABSTRACT

Objective:

To systematically review the literature to verify the validity of field-tests to evaluate cardiorespiratory fitness (CRF) in children and adolescents.

Data sources:

The electronic search was conducted in the databases: Medline (PubMed), SPORTDiscus, Scopus, Web of Science, in addition to the Latin American databases LILACS and SciELO. The search comprised the period from the inception of each database until February 2015, in English and Portuguese. All stages of the process were performed in accordance with the PRISMA flow diagram.

Data synthesis:

After confirming the inclusion criteria, eligibility, and quality of the studies, 43 studies were analyzed in full; 38 obtained through the searches in the electronic databases, and 5 through private libraries, and references from other articles. Of the total studies, only 13 were considered high quality according to the adopted criteria. The most commonly investigated test in the literature was the 20-meter shuttle run (SR-20 m), accounting for 23 studies, followed by tests of distances between 550 meters and 1 mile, in 9 studies, timed tests of 6, 9, and 12 minutes, also 9 studies, and finally bench protocols and new test proposals represented in 7 studies.

Conclusions:

The SR-20-m test seems to be the most appropriate to evaluate the CRF of young people with the equation of Barnett, recommended to estimate VO2 peak. As an alternative for evaluating CRF, the 1-mile test is indicated with the equation proposed by Cureton for estimating VO2 peak.

INTRODUÇÃO

A aptidão física, em geral, refere-se a uma série de características físicas que estão diretamente relacionadas à capacidade de uma pessoa realizar atividades físicas e/ou se exercitar.11 Ortega F, Ruiz J, Castillo M, Sjöström M. Neste sentido, dentre seus componentes destaca-se a aptidão cardiorrespiratória (ACR), também conhecida como aptidão aeróbica ou potência aeróbica máxima.22. Armstrong N. Atualmente, a ACR tem sido considerada um importante marcador de saúde de adultos33. Blair SN, Wei M, Lee CD. Crianças e adolescentes que possuem altos níveis nos indicadores cardiorrespiratórios tendem a apresentar menos fatores de risco para doenças cardiovasculares, como obesidade, pressão alta, dislipidemia, e resistência à insulina, entre outros.66. Além disso, estudos prospectivos indicaram que a alta ACR durante a infância e adolescência está associada à um perfil cardiovascular saudável na vida adulta.77. Com relação à avaliação dos indicadores da ACR, o consumo máximo de oxigênio (VO2pico) é reconhecido como um dos melhores índices para medir a potência aeróbica entre os jovens.22. Armstrong N. Aerobic fitness of children and adolescents. J Pediatr. 2006;82:406-8. O VO2pico pode ser medido objetivamente e de maneira confiável em laboratório, por meio da análise direta dos gases envolvidos na ventilação pulmonar, durante a execução de testes progressivos e máximos em diversos ergômetros. Porém, devido ao alto custo, uso de equipamentos sofisticados, necessidade de avaliadores capacitados para realizar os testes e a grande demanda de tempo para cada avaliação, seu uso fica limitado em ambientes como escolas, clubes esportivos e estudos com a população.88. Dessa forma, a aplicação de testes baseados em campo, que proporcionam a predição do VO2pico por meio de modelos matemáticos têm se tornado uma alternativa interessante para avaliar a ACR, já que demonstram vantagens significativas, como baixo custo operacional, facilidade na aplicação e acesso aos locais de teste, assim como a oportunidade de avaliar um grande número de pessoas ao mesmo tempo.99. Por outro lado, testes de campo para a avaliação da ACR usam métodos indiretos para estimar o VO2pico, ou seja, podem apresentar erros de medição. Então, para que um teste de campo seja considerado apropriado, deve ter boa “validade”, ou seja, produzir boas medidas da variável que se propõe a medir. Dessa forma, ao escolher um protocolo de campo dentre os propostos na literatura para avaliar a ACR, é importante verificar se este é válido para a população analisada.

Duas décadas após a primeira iniciativa que sintetizou os critérios relacionados à validação dos vários testes de campo para avaliar a aptidão física entre os jovens, Castro-Piñero et al.88. Castro-Piñero J, Artero EG, España-Romero V, Ortega FB, Sjöström M, Suni J, et al. Criterion-related validity of field-based fitness tests in youth: a systematic review. Br J Sports Med. 2010;44:934-43. atualizaram esta revisão, de forma mais criteriosa e sistemática, considerando os diferentes níveis de evidência para a validade dos testes de campo, de acordo com os critérios de qualidade estabelecidos para os estudos analisados. Assim, considerando a velocidade na produção da literatura científica atual, mostrando novas evidências de validade, principalmente na infância e na adolescência, que são fases importantes para a detecção de problemas de saúde e para promover intervenções, este tipo de estudo torna-se necessário.

Desta forma, o objetivo deste estudo foi de fazer uma revisão sistemática da literatura para verificar a validade de testes de campo para avaliação da ACR em crianças e adolescentes.

MÉTODO

Realizamos uma revisão sistemática da literatura usando as bases MedLine (PUBMED), SPORTDiscus, Scopus, Web of Science, além das bases de dados latinoamericanas, LILACS, e SciELO. A busca compreendeu todo o período de existência das bases até fevereiro de 2015, em idioma inglês e português. Decidimos usar somente essas duas línguas porque os principais estudos estavam disponíveis em inglês; incluímos o português porque achamos importante demonstrar esta informação.

A estratégia de busca compreendeu as seguintes palavras-chave e operadores booleanos: (“Validation Studies” OR “Cross Validation” OR “Criterion Validity” OR “Validity” OR “Reliability” OR “Comparison”) AND (“Oxygen Consumption” OR “Maximal Oxygen Uptake” OR “Cardiorespiratory Fitness” OR “Aerobic Capacity” OR “Aerobic Fitness” OR “VO2 Peak” OR “VO2 Maximal” OR “Field Test” OR “Shuttle Run Test” OR “Walk Test” OR “Run Test” OR “SR-20m” OR “20m Multistage Shuttle Run” OR “12 Minute” OR “One Mile Walk Test” OR “1600m Run”) AND (“Child” OR “Children” OR “Teenager” OR “Youth” OR “Adolescent” OR “Adolescents” OR “Students” OR “Schoolchildren”). Foram aplicados os filtros: busca por título, palavras-chave, resumo e/ou todos os campos/assuntos; artigos científicos publicados em periódicos, incluindo artigos de revisão e revisão sistemática; área de estudos e que envolvessem seres humanos. Para base de dados Scopus, ainda foi utilizado o filtro específico para área de interesse, na qual foi selecionado “Profissões da Saúde”.

No caso específico das bases de dados latino-americanas LILACS e Scielo, foram utilizadas as seguintes palavras chave para busca: (“jovens” OR “adolescentes”) AND (“aptidão cardiorrespiratória” OR “teste de 9 minutos” OR “consumo de oxigênio”).

Os critérios de elegibilidade dos artigos foram: ter como objetivo principal a investigação da validade de um ou mais testes de campo para estimativa da ACR e, a população investigada ser de crianças e/ou adolescentes considerados saudáveis, ou seja, sem qualquer patologia diagnosticada ou eventuais problemas que impedissem a realização de testes motores e não-atletas.

Todas as fases do processo foram realizadas de acordo com o fluxograma do PRISMA1010. Liberati A, Altman DG, Tetzlaff J, Mulrow C, Gøtzsche PC, Ioannidis JP, et al. (Figura 1) e a seleção e análise dos estudos foram realizadas de maneira independente, por dois pesquisadores (MBB e CLPR). No caso de discordância, um terceiro pesquisador (ERVR) era convidado a determinar a inclusão ou exclusão de estudos.

Após a conclusão da busca, de acordo com os procedimentos já demonstrados, 3.197 artigos foram localizados nas seis bases de dados analisadas. Cinco estudos extras foram localizados e incluídos de bibliotecas privadas e referências bibliográficas. O passo seguinte do processo foi a exclusão de referências duplicadas, e 736 referências foram excluídas, ou seja, 2.461 títulos foram analisados. O passo a seguir consistiu da leitura dos títulos dos artigos selecionados para possível exclusão daqueles que não estavam adequados aos critérios de elegibilidade; 2.287 artigos foram excluídos, restando 174.

Os resumos foram lidos para analisar os critérios, assim como para a inclusão e exclusão dos estudos que geraram dúvidas durante a leitura do título. Cento e cinco estudos foram excluídos por não estarem de acordo com os critérios de elegibilidade, deixando 69 estudos para a próxima fase. Então, outros 25 artigos foram excluídos por outras razões, deixando, então, 44 estudos.

A análise qualitativa foi adaptada de Castro-Piñero et al.88. Castro-Piñero J, Artero EG, España-Romero V, Ortega FB, Sjöström M, Suni J, et al. Criterion-related validity of field-based fitness tests in youth: a systematic review. Br J Sports Med. 2010;44:934-43., e considerou três itens nos artigos: número de participantes, descrição da amostra e análise estatística. Para cada um desses itens, o artigo poderia receber uma pontuação de 0 a 2, e, ao final da análise, uma classificação foi dada de acordo com a soma dos pontos de cada item. Uma adaptação da pontuação para qualidade foi realizada e categorizada da seguinte forma: qualidade baixa (0-2 pontos), moderada (3-4 pontos) ou de alta qualidade (5-6 pontos). Depois desta avaliação, seis artigos foram excluídos devido à baixa qualidade metodológica.

Por meio da análise qualitativa dos estudos incluídos nesta revisão sistemática, foi possível estabelecer níveis de evidência relacionados à validade dos protocolos do estudo. Uma evidência forte foi atribuída a protocolos de teste considerados válidos por 3 ou mais estudos de alta qualidade; a evidência moderada foi atribuída a testes validados por 2 estudos de alta qualidade ou 3 ou mais estudos de qualidade moderada; e evidência limitada foi atribuída a testes validados por vários estudos de baixa qualidade, resultados inconsistentes de diversos estudos, independentemente da qualidade, ou os resultados de um único estudo.

RESULTADOS

O processo de seleção do estudo está demonstrado na Figura 1. A busca preliminar encontrou 3.197 artigos, sendo 968 na base de dados Medline (PUBMED), 349 na Web of Science, 1.314 no Scopus, 321 no Sport Discus, 53 na SciELO e 192 na Lilacs.

Após a análise dos critérios de inclusão e elegibilidade, um total de 3.153 estudos foram excluídos até esta fase do processo. Depois, os artigos foram classificados de acordo com critérios qualitativos. Este procedimento foi adotado para garantir que somente estudos que tivessem qualidade metodológica pelo menos moderada fossem incluídos e, assim, permitir que ao final do processo de revisão sistemática pudessem ser identificados níveis de evidências quanto à validade dos testes de campo analisados.88. Além disso, como as bases de dados latino-americanas foram inseridas na busca, usamos a evidência moderada já que alguns testes são muito usados no Brasil; por exemplo, o teste de corrida/caminhada de 9 minutos não tinha nenhuma validação.

Porém, além de determinar o VO2pico com o teste SR-20m, uma alternativa mais simples e muito usada por profissionais é a verificação somente dos parâmetros atingidos no teste, como o número de voltas (indo e voltando) e a velocidade alcançada na fase final do teste. O participante deve correr de uma linha a outra em intervalos de 15 segundos correndo e 15 segundos descansando, para completar a maior distância possível até o final dos 10 minutos. Os autores validaram o teste (r=0,68) e também propuseram uma equação de previsão para o VO2 pico considerando a distância máxima alcançada no teste em metros, e por sexo (r=0,84).4444. Porém, é preciso ter cuidado ao interpretar os resultados da validação cruzada das equações mencionadas, já que, na maioria das vezes, os resultados foram satisfatórios em análises de grupo checadas pela ANOVA, pelo coeficiente de correlação linear e pela regressão linear simples, mas não quando verificadas no nível individual, por meio do acordo promovido pela análise de Bland e Altman em 1986, nem com relação à parcialidade e tendências. Assim, o pesquisador deve escolher a equação mais adequada de acordo com seu objetivo; análises de grupo ou individual.

Testes de corrida e/ou caminhada em distâncias de 550 m a 1.600 m (1 milha)

Dentre os protocolos que consideram distâncias pré-estabelecidas, o teste de corrida/caminhada de 1 milha é o mais divulgado e analisado na literatura, sendo usado em um total de 5 artigos que estavam de acordo com os critérios de inclusão e elegibilidade desta revisão.1616. Em relação aos testes com distâncias pré-estabelecidas, há evidência moderada para o teste de caminhada/corrida de 1 milha, e evidênica limitada para protocolos da corrida de 550 metros, caminhada de 1 milha, teste submáximo de 1 milha, e corrida/caminhada de meia milha devido à falta de estudos que validem esses testes entre os jovens. Testes de corrida/caminhada de 6, 9 e 12 minutos

No teste de caminhada de 6 minutos, houve evidência limitada de validade, com resultados inconsistentes nos estudos. Isso também foi demonstrado no teste de corrida de 6 minutos, sendo que somente uma análise testou sua validade.2424. Van Mechelen W, Hlobil H, Kemper HC. Este fato pode ser explicado parcialmente pelas características do teste, como sua duração e tipo de esforço. Por outro lado, o teste de 9 minutos apresentou evidência de validade moderada. Assim como os testes de corrida e caminhada de 6 minutos, o de 12 minutos também demonstrou evidência de validade limitada entre os jovens, representada somente em dois estudos de qualidade moderada que verificaram sua validade,3939. Assim, sugerimos outras iniciativas para verificar a validade de protocolos de campo em testes de corrida e/ou caminhada de 6 e 12 minutos, para promover resultados mais consistentes na população de crianças e adolescentes.

Porém, outras iniciativas de validação cruzada são necessárias para provar seu uso em populações diferentes. Assim, há evidência moderada de validade para o teste de banco, mas é importante ressaltar que os testes demonstraram diferenças, de acordo com o protocolo usado.

Então, é preciso ter cuidado ao usar o teste de Andersen et al.,4444. Andersen LB, Andersen TE, Andersen E, Anderssen SA. J Sports Med Phys Fitness. 2008;48:434. já que foi destinado a uma pequena amostra de pessoas de 9 a 11 anos de idade, e, portanto, mesmo com moderada evidência de validade, necessita ser testado em amostras de outras idades antes de ser utilizado.

CONCLUSÕES

Diante dos resultados encontrados nesta revisão sistemática, a conclusão é de que o teste SR-20 m aparentemente é o mais adequado para avaliar a ACR entre os jovens, apresentando forte evidência de validade. A equação recomendada para estimar o VO2pico com o teste SR-20 m é aquela proposta por Barnett et al.3131. Barnett A, Chan LY, Bruce IC. Mahar MT, Guerieri AM, Hanna MS, Kemble CD. Estimation of aerobic fitness from 20-m multistage shuttle run test performance. Am J Prev Med. 2011;41 Suppl 2:S117-23., devido à evidência moderada de validade, sendo recomendada pela bateria FITNESSGRAM para estimar o VO2pico.

Como possível alternativa para avaliar a ACR, quando não for possível usar o teste SR-20 m, indica-se o teste de 1 milha, que demonstrou evidência moderada de validade, assim como a equação proposta por Cureton et al.3333. J Sports Med Phys Fitness. 2008;48:434. que apresentaram moderada evidência de validade, porém, sem indicação de equações para estimativa do VO2pico por meio desses testes até o momento.
