ABSTRACT

There are hundreds of operations described for the treatment of hallux valgus, which itself demonstrates that no single operation addresses the varied spectrum of hallux valgus presentation. Gait is examined for restoration of medial arch during toe-off position. The 1st metatarsal osteotomy involves translation of the distal fragment laterally to reposition the metatarsal head over the sesamoids. The main complication of the Chevron osteotomy is avascular necrosis of the 1st metatarsal head which is seen in up to 20% of cases in certain studies especially if simultaneous lateral release is performed. The main complication is guttering of the two osteotomy fragments leading to dorsal displacement of the 1st metatarsal head fragment and increasing the risk of transfer metatarsalgia. Dorsal closing wedge osteotomy of the base of the proximal phalanx helps shift the arc of motion of the great toe metatarsophalangeal joint allowing more dorsiflexion.
