2012, Child and Adolescent Psychiatric Clinics of North AmericaCitation Excerpt :A smaller study of 33 adolescents with FAP randomized to amitriptyline or placebo over 13 weeks found improvements in quality of life for the amitriptyline group compared with placebo, but was compromised by methodologic weaknesses that included multiple testing and small sample size.56 Past reports of sudden death in association with the use of TCAs in children, their potential for lethality in overdose, and the absence of proven efficacy in the treatment of both FAP and pediatric depression57 make it difficult to confidently recommend TCAs for youth with anxiety and FSS. Although there are numerous antidepressant medications worthy of consideration in the management of anxiety in the presence of physical symptoms, including venlafaxine and duloxetine, mirtazapine is a novel antidepressant that may be of special interest, particularly for patients with gastrointestinal complaints, given its ability to block the serotonin 5-HT3 receptor relevant to nausea and gastrointestinal discomfort.50
