Identification of jeopardized myocardium

Gated myocardial perfusion SPECT imaging provides information on both the amount of jeopardized myocardium and the size of irreversibly damaged myocardium. The combined assessment of perfusion and function improves the prognostic power of nuclear stress testing and enhances the precision in stratifying patients into cardiac event risk levels. Quantitative assessment of regional wall motion

Semiquantitative visual assessment of regional myocardial function requires a great deal of experience in interpretation of gated SPECT images. Two parameters should be evaluated: endocardial inward motion and wall thickening. Whereas segmental motion is best evaluated on a gray-scale display, wall thickening is better assessed as brightening on a colored scale, taking advantage of the systolic increase in count density as a function of wall thickness, as a result of the partial-volume effect.

    LBBB patients with an EDV of 160 mL or greater, an ESV of 100 mL or greater, or an LVEF of less than 35% were at increased risk for subsequent cardiac events. Our results underscore previously published data on the prognostic value of gated SPECT imaging.22-26 Sharir et al22 showed that perfusion variables and ESV were powerful markers in the prediction of total coronary events, whereas in the prediction of cardiac death, poststress LVEF and ESV were independent predictors and had incremental value over perfusion data.
