Epidemiología de la fibrilación auricular
La fibrilación auricular (FA) es la arritmia cardiaca sostenida más frecuente, con una prevalencia estimada del 4,8% en la población española1 y con una estrecha asociación con la edad2, 3. Uno de los principales retos asociados al manejo de la FA ha sido el de reducir el riesgo de ET. La profilaxis antitrombótica se ha demostrado eficaz10, 11, pudiendo reducir su probabilidad desde un 20% mediante el tratamiento antiagregante con ácido acetilsalicílico (AAS) hasta un 64% con tratamiento anticoagulante, como es el caso de warfarina12, lo que ha supuesto la generalización de los antivitamina K (AVK) como tratamiento de referencia. En este contexto surgen los nuevos anticoagulantes orales (NACO), actualmente denominados anticoagulantes de acción directa (AAD). A día de hoy no existe ninguna comparación directa entre AAD, lo que dificulta la elección de uno frente a otro. En el último año se han publicado metaanálisis y comparativos con datos agrupados, pero no podemos obviar sus limitaciones. El riesgo trombótico de los pacientes entre los diferentes ensayos no fue uniforme, como no lo fue el tiempo de seguimiento ni el control del grupo warfarina (las características de los 4 ensayos se resumen en la tabla 2b). Más allá de las conclusiones globales que ofrecen estos estudios, el manejo de estos fármacos podría verse condicionado por determinadas situaciones clínicas.
La edad, que implica una mayor fragilidad en la mayoría de los casos, podría ser uno de los principales condicionantes para el inicio de los AAD. Así, los pacientes mayores de 75 años tratados con dabigatrán o rivaroxabán podrían tener mayor riesgo de hemorragia grave en comparación con AVK30, 31, aunque su eficacia no se vería alterada.
Experiencia práctica clínica
Aunque el uso de AAD es aún escaso, tal y como se recoge en el registro GARFIELD23, es esperable que su generalización ponga de manifiesto nuevas problemáticas al ser introducidos en una población real. Existen a día de hoy pocos estudios acerca de la experiencia de su uso. Recomendaciones
Los AAD han entrado con fuerza en las principales guías de referencia en el manejo de FANV. Se sitúan al mismo nivel que los AVK en pacientes con indicación de anticoagulación14, 16, e incluso se sugiere la posibilidad, aunque con menor grado de evidencia, de que sean la primera opción de tratamiento. Uno de los condicionantes para la implantación definitiva de AAD es su coste. Con un precio mucho más elevado que el de AVK, es importante realizar estudios que consideren todas las variables para una toma de decisiones prudente. Existen aún dudas acerca de que los AAD supongan un beneficio en términos de coste frente a AVK49. La FA sigue siendo un importante reto tanto a nivel sanitario como económico. Sigue existiendo un elevado número de pacientes que no cuentan con un tratamiento optimizado, en muchas ocasiones por temor a las complicaciones derivadas de un tratamiento anticoagulante. Los AAD suponen un gran avance en la profilaxis antitrombótica.
