Key points

  • Reoperation rates are higher in total ankle arthroplasties (TAAs) compared with ankle arthrodesis. Infection rates for primary TAAs are 1.4% to 2.4%.

  • The survival rate of TAA is approximately 75% to 90% at 10 years.

  • Arc of motion is maintained with TAAs compared with ankle arthrodesis. Ankle arthrodesis increases arc of motion through the talonavicular joint, which is a cause for concern for adjacent joint disease in the future.

  • Several factors are strong reasons to favor ankle fusion rather than

Ankle arthrodesis

At present, the most accepted surgical treatment of end-stage ankle arthritis is ankle arthrodesis. The procedure is straightforward and is suitable for surgical treatment of almost all types of ankle arthritis. There exists ample evidence in the literature ranging from level I to IV studies to support a grade A recommendation (good evidence with consistent findings from level I studies) or at least a grade B recommendation (fair evidence with consistent level II or III studies) for the use of

Ankle arthroplasty

Ankle arthroplasty is growing in popularity for the treatment of end-stage ankle arthritis. As surgical techniques and implants improve, the clinical results of ankle arthroplasty do so as well. Note that it is still a very technical operation and surgeons’ experience has been shown to influence complication rates.

Summary

TAA has reemerged as a viable and often preferable option to ankle arthrodesis for patients with end-stage ankle disease so it is vital to understand how patients should be counseled. The decision between ankle arthroplasty and arthrodesis should be made on a case-by-case basis. There are no consensus guidelines, but a recent decision analysis using a Markov model suggested that TAR was a superior option.57 However, this model does not account for patient-specific risk factors.
