The Second International Conference on Surgical Rehabilitation of the Upper Limb in Tetraplegia (Quadriplegia)

Nous rapportons les résultats de la comparaison d’un programme de chirurgie fonctionnelle du membre supérieur chez des patients tétraplégiques comprenant une réanimation de l’extension du coude et la construction d’une pince pollicidigitale en « un seul temps » opératoire ou en « deux temps » opératoires.

Une série de 36 patients tétraplégiques avec 45 membres supérieurs opérés a été analysée rétrospectivement avec un recul moyen de 23 mois (13–39 mois). Nous avons évalué la force analytique de l’extension du coude à l’aide du score MRC (Medical Research Council), la force de la pince pollicidigitale (Kg/F) ainsi que l’autonomie des patients à l’aides de scores fonctionnels : Lamb (/100) et QIF (Quadriplegia Infex of Function) (/100).

Seize membres supérieurs ont été inclus dans le groupe « un temps » et 29 dans le groupe « deux temps ». Les groupes étaient comparables en pré opératoire. La force d’extension du coude était de 3,6/5 dans le groupe « un temps » contre 3,7/5, p = 0,74. La force de pinch était de 1,8 kg/F dans le groupe « un temps » contre 1,3 kg/F, p = 0,22. Concernant les scores fonctionnels, il n’existait pas de différence statistiquement significative entre les deux groupes pour le test Lamb (p = 0,46), le QIF (p = 0,42). Il n’y avait pas de différence statistiquement significative en termes de fréquence de complications sur le coude (p = 0,49) et sur la main (p = 0,17). La durée totale du programme « un temps » était en moyenne de 7,5 mois contre 24,7 mois pour le programme « deux temps » p < 0,01. Le recul moyen du groupe « un temps » était de 21 mois contre 24 mois pour le groupe « deux temps ».

Les résultats des deux programmes sont similaires et font du programme en « un temps » une option fiable. L’option chirurgicale tout en un temps permettant de d’alléger les suites post opératoires et les taux d’abandon des patients en cours de programme.

IV série clinique rétrospective.

We report the comparative results of functional upper extremity surgery plans in patients with tetraplegia, including restoration of elbow extension and construction of a key grip in a “one-stage” or “two-stage” operation.

A series of 36 patients with tetraplegia, with a total of 45 operated upper limbs, was analyzed retrospectively with a mean follow-up of 23 months (13–39 months). We evaluated the analytical strength of elbow extension using the MRC (Medical Research Council) score, the strength of the key grip (kg/F) as well as the autonomy of the patients using functional scores; Lamb (/100) and QIF (Quadriplegia Index of Function) (/100).

Sixteen upper limbs were included in the “one-stage” group and 29 in the “two-stage” group. The groups were comparable preoperatively. Elbow extension strength was 3.6/5 in the one-stage group versus 3.7/5, p = 0.74. Grip strength was 1.8 kg/F in the one-stage group versus 1.3 kg/F, p = 0.22. Regarding the functional scores, there was no statistically significant difference between the two groups for the Lamb score (p = 0.46) and the QIF (p = 0.42). There was no statistically significant difference in terms of the frequency of complications for the elbow (p = 0.49) and for the hand (p = 0.17). The total duration of the one-stage surgery plan averaged 7.5 months compared to 24.7 months for the two-stage plan, p < 0.01. The mean follow-up for the one-stage group was 21 months versus 24 months for the two-stage group.

The results of both plans are similar and indicate that the “one-stage” plan is a reliable option. This all-in-one surgical option reduces postoperative follow-up and dropout rates for patients during the plan timeframe.

IV; retrospective clinical series.

This study determines the clinical utility of patient-reported outcome measures used to measure outcomes of upper extremity (UE) reconstructive procedures in individuals with tetraplegia. The patient-reported outcome measures are the Canadian Occupational Performance Measure, the Capabilities of Upper Extremity Questionnaire (CUE-Q), and the Personal Wellbeing Index.

Retrospective data of 43 individuals with spinal cord injury (SCI) levels C4-C7 tetraplegia, and American Spinal Injury Association Impairment Scale grades A-D who had upper limb reconstructive surgery were reviewed. Participants were grouped according to their SCI level and resultant surgical procedures into higher SCI severity and lower SCI severity groups.

The mean age of participants was 26.3 years (SD 13.4; range 13–64 years). The higher-severity SCI group required elbow and hand reconstruction surgery, whereas the lower-severity group only required hand reconstruction surgery. Important differences in Canadian Occupational Performance Measure priorities were identified between the higher and lower SCI severity groups. Question redundancy was evident with the CUE-Q. The self-report Personal Wellbeing Index captures the possible impacts of improved UE function on an individual’s perceived sense of personal wellbeing.

In this patient-reported outcome measure analysis, we found that the level of impairment influences patient priorities. Functional measures ought to consider UE impairment and personal wellbeing as a construct in this population, given the demands of surgery.

Prognostic II

In spastic patients, shortening wrist arthrodesis (SWA) is indicated in cases of severe fixed flexion contracture. At present, the most commonly used technique is dorsal plate osteosynthesis. Ideally, fixation with smaller hardware volume farther from the tendons would limit postoperative tendon irritation and reoperation rates for hardware removal. The objective of our study was to evaluate the efficacy of Rush pin SWA in adults. A retrospective study included all patients with a central neurological impairment, undergoing SWA using a Rush pin inserted through the head of the third metacarpal, and with at least 6 months’ follow-up. Attainment of preoperative objectives was evaluated by Global Assessment of Response to Treatment (GART, ranging from −4 to +4) and, for functional objectives, the House score and the Frenchay Arm Test. Consolidation and any degenerative changes in the third metacarpophalangeal joint were assessed on X-ray. Fifteen patients were included, with a mean follow-up of 13 months (range, 6–29). In general, the preoperative objectives were attained: mean GART score was 2.7 (range, 1–4). Functional objectives were attained in 3 of the 11 patients followed up (27%). In all cases, the arthrodesis had healed at a mean 74 days (range, 39–102). Three had hardware removed after consolidation; 1 experienced discomfort at the head of the third metacarpal. Rush pin arthrodesis is an interesting alternative to plate arthrodesis in the management of severe wrist flexion contracture in spastic patients. It gives satisfactory results with regard to preoperative objectives and is not associated with complications.

IV, retrospective study without control group.

L’arthrodèse raccourcissante du poignet (ARP) chez l’adulte spastique est indiquée en cas de flessum majeur fixé. La technique la plus utilisée actuellement est l’ostéosynthèse par plaque par voie dorsale. Une ostéosynthèse moins volumineuse à distance des tendons limiterait l’irritation tendineuse postopératoire et les taux de réintervention pour ablation du matériel. L’objectif de notre étude était d’évaluer les résultats de l’ARP par clou de Rush chez les patients spastiques adultes. Dans une étude rétrospective, ont été inclus les patients présentant une atteinte neurologique centrale, ayant bénéficié d’une ARP à l’aide d’un clou de Rush introduit par la tête du troisième métacarpien, avec un recul minimum de 6 mois. L’objectif préopératoire était relevé et sa satisfaction était évaluée par le Global Assessment of Response to Treatment (GART) variant de −4 à +4. L’atteinte de l’objectif fonctionnel était évaluée par le score de House ou le Frenchay Arm Test (FAT). Une radiographie était réalisée afin d’évaluer la consolidation et de chercher des remaniements dégénératifs de la troisième articulation métacarpo-phalangienne. Quinze patients étaient inclus avec un recul moyen de 13 mois (6–29). Il existait une amélioration de l’objectif fixé en préopératoire avec un GART global moyen à 2,7 (1–4). Trois patients sur 11 (27%) avaient vu une amélioration de leur fonction en postopératoire. Tous les patients avaient consolidé à un délai moyen de 74 jours (39–102). Trois patients avaient bénéficié d’une ablation de matériel après consolidation, seul 1 patient présentait une gêne en regard de la tête du 3^ème métacarpien. L’arthrodèse par clou de Rush est une alternative intéressante à l’arthrodèse par plaque dans la prise en charge des flessum sévères de poignet chez les patients spastiques et apporte des résultats satisfaisants en termes d’objectifs fixés en préopératoire sans complication associée.

This chapter examines the current evidence-based rehabilitation strategies for management of individuals with traumatic spinal cord injury (SCI). The chapter begins with a brief review of the history of SCI rehabilitation and subsequently reviews current innovative and somewhat controversial rehabilitation strategies for restoration of upper- and lower-extremity motor and sensory function after SCI. It then describes the key determinants for the outcomes after rehabilitation of individuals with traumatic SCI. The chapter closes with a review of secondary health conditions after SCI with an emphasis on the importance of their prevention, early recognition, and effective management to prevent rehabilitation service interruptions and augment adherence. Major knowledge gaps and opportunities for future research are highlighted given the nature of traumatic SCI.

The author relates his 40 years of experience in the field of functional surgery of the tetraplegic upper limb (FSTUL). After having introduced and developed this specialty in France, he disseminated it to various countries where he saw a large number of patients and acquired extensive experience. He presents his personal progression and discusses the recent therapies in this field. FSTUL is a personalize surgery, with each case being unique. It must, first and foremost, take into consideration the real wishes of a motivated and well-informed patient and consider the type of spinal cord injury (not only the upper level of the lesional segment but also the extent of the sub-lesional segment). The surgical indication and rehabilitation are very important factors in the outcome. The main goal of FSTUL is rapid social reintegration of the patient; in some cases, this could involve fewer surgical procedures and shorter stays in rehabilitation centers. FSTUL is in constant evolution and can be improved; it is a useful but not well-known surgery.

L’auteur relate ses activités durant 40 ans concernant la chirurgie fonctionnelle du membre supérieur du tétraplégique (CFMST). Après avoir introduit et développé cette spécialité en France, il en a fait de même dans d’autres pays, lui permettant de voir de nombreux malades et d’acquérir ainsi une importante expérience. Il expose son évolution personnelle et discute des nouveautés apparues en ce domaine. La CFMST est une chirurgie personnalisée adaptée à chaque cas. Elle doit tenir compte: 1) Avant tout des désirs réalistes d’un patient motivé et informé. 2) Du type de la lésion médullaire (non seulement de son niveau supérieur, mais aussi de l’étendue des segments lésionnel et sous-lésionnel). 3) L’indication opératoire et la réhabilitation sont de première importance. Le but de la CFMST est une rapide réinsertion socioprofessionnelle nécessitant, si possible, la réduction du nombre des temps opératoires et la durée des séjours en rééducation. La CFMST est en constante évolution et peut être améliorée mais est actuellement une chirurgie très utile et insuffisamment connue.
