Evaluating quality-of-life measures for clinical trials in Germany

Abstract

In spite of an increased discussion of psychosocial outcomes of medical treatments, quality-of-life assessments in clinical trials conducted in Germany are rare. Barriers to the development of quality-of-life-related evaluation research include conceptual, methodologic, practical, and political issues. Each issue is characterized by specific problem areas and by current approaches to problem solving; both are affected by crosscultural considerations. This paper analyzes state-of-the-art in quality-of-life research in Germany with regard to four key issues and presents original work in a biometrical center as an example.

Transfusion in elderly: Take account frailty

La conjonction du vieillissement démographique et de l’augmentation de la fréquence de l’anémie avec l’avancée en âge font que le nombre de concentrés globulaires délivrés chaque année augmentent avec comme conséquence une forte pression sur la collecte de sang. Les étiologies de l’anémie du sujet âgé sont souvent multifactorielles et leur recherche est une étape indispensable et préalable à tout traitement. Les seuils transfusionnels, en particulier chez le sujet âgé, évoluent progressivement et une stratégie dite restrictive est désormais privilégiée. Les complications immédiates et différées de l’acte transfusionnel sont plus fréquentes chez le sujet âgé du fait de facteurs de vulnérabilité liés à la fragilité et du risque de transfusions multiples. Le repérage des complications est essentiel et permet d’éviter leur récurrence. L’impact de la transfusion sur la qualité de vie des sujets âgés n’est pas démontré et représente un sujet de controverse. En outre, la transfusion de concentrés de globules rouges (CGR) s’accompagne d’une hausse des dépenses de santé et d’une augmentation de la morbi-mortalité, dont les risques peuvent être réduits grâce à des alternatives à la transfusion. Des études longitudinales incluant des sujets âgés permettraient de mieux cerner les enjeux de la transfusion de CGR dans cette population.
The conjunction of the demographic aging and the increase in the frequency of anemia with the advancing age, mean that the number of globular concentrates delivered each year increases with a consequent heavy pressure on blood collection. The etiologies of anemia in the elderly are often multifactorial and their investigation is an indispensable step and prior to any treatment. Transfusion thresholds, particularly in the elderly, are gradually evolving and a so-called restrictive strategy is now favored. Immediate and delayed complications of transfusion are more frequent in the elderly due to vulnerability factors associated with frailty and the risk of multiple transfusions. The screening of complications related to transfusion of RBCs is essential and makes it possible to avoid their recurrence. The impact of transfusion on the quality of life of elderly patients is not obvious and is a controversial issue. In addition, transfusion of red blood cells (RBCs) is accompanied by an increase in health expenditure and an increase in morbidity and mortality, whose risks can be reduced through alternatives to transfusion. Longitudinal studies, including elderly subjects, would allow a better understanding of the issues involved in the transfusion of RBCs in this population.
Quality of life in patients with myelodysplastic syndromes

Myelodysplastic syndromes severely affect the quality of life (QOL). Until recently, available treatments, at least for the older patients, have been unsatisfactory and largely symptomatic. Having little impact on prognosis, the benefit of therapy on symptomatic aspects and QOL had to be ascertained. Erythropoesis stimulating factors like erythropoietin and darbepeotin have been shown to improve QOL in subgroups of MDS patients temporarily. QOL is now discussed in the context of the new hypomethylating agents and lenalidomide. Improvements in haemoglobin levels are associated with gains in QOL, which are not offset by the manageable side effects of these therapies. However, dealing with the emotional burden of a life threatening disease and the related uncertainty about the future remain a challenge in the interaction with these patients.
