Resumen

Introducción

La fibromialgia constituye un grave problema de salud por la afectación de las capacidades funcionales de los pacientes, su elevada prevalencia, los costos de su tratamiento, y el deterioro de la calidad de vida individual y familiar.

Objetiv

o Describir la calidad de vida relacionada con la salud (CVRS) y su asociación con aspectos demográficos, socioeconómicos y de salud, en personas con fibromialgia de Medellín.

Materiales y métodos

Estudio transversal en 100 adultos con fibromialgia. Se aplicó una encuesta y el FIQ (Fibromyalgia Impact Questionnaire). El análisis se realizó con proporciones, medidas de resumen, intervalos de confianza, t Student, U de Mann-Whitney, H de Kruskal-Wallis, ANOVA y regresión lineal múltiple.

Resultados

92% fueron mujeres, la prevalencia de comorbilidad fue de 81%. En las dimensiones del FIQ la función física presentó el resultado más favorable, mientras que los puntajes más deficientes se registraron en dolor, fatiga y sentirse bien; el puntaje global de CVRS fue de 60,5. Algunos factores asociados a la CVRS fueron la edad, el ingreso económico, la escolaridad, la ocupación y el régimen de afiliación en salud. Según los modelos de regresión, la salud percibida es el factor más relevante para la CVRS en fibromialgia, y las variables incluidas en cada dimensión del FIQ presentaron coeficientes de determinación entre 14% y 39%.

Conclusión

La CVRS fue baja en el grupo de estudio, su medición resulta de gran relevancia para diseñar, implementar, seguir y evaluar programas de prevención, atención y rehabilitación, y complementar indicadores tradicionales de morbilidad, mortalidad y expectativa de vida.

Summary

Introduction

Fibromyalgia is public health problem for the impact in of the functional capabilities of patients, its high prevalence, treatment costs and the deterioration of the quality of life of the individual and his family.

Objective

To describe the health related quality of life (HRQOL) and its association with demographic, socioeconomic and clinical conditions in adults with fibromyalgia in Medellín.

Materials and methods

: Cross-sectional study in 100 adults with fibromyalgia. A survey and FIQ were applied. The analysis was performed with proportions, summary measures, confidence intervals, t Student, U Mann-Whitney, Kruskal-Wallis, ANOVA and multiple linear regression.

Results

92% were women, the prevalence of comorbility was 81%. The dimensions of the FIQ Physical Function presented the most favorable outcome, while poorer scores were in Pain, Fatigue and Feeling good, global HRQoL score was 60,5. Some factors associated with HRQoL were age, income, education, occupation and health system affiliation. According to the regression models the perceived health is the most important factor for HRQOL in fibromyalgia and the variables included in each dimension of FIQ showed coefficients of determination between 14% and 39%.

Conclusion

HRQL was low in the study group, its measurement is of great importance to design, implement, monitor and evaluate programs for prevention, care and rehabilitation, and complement traditional measures of morbidity, mortality and life expectancy.

    FMS patients exhibited markedly lower scores on the SF-36 than healthy individuals. In line with previous studies, the reduction in HRQoL manifested at physical, psychological and social levels of functioning, and in all domains and components of the SF-36 (Cardona-Arias et al. 2012; Jeong-Won et al. 2017).     Another SF-36-based study involving a sample of Colombian SLE patients demonstrated a QOL deterioration produced by the disease, pain being the symptom having the greatest impact (without reducing anxiety and depression symptoms’ importance).26 Our EQ-5D-3L results revealed that FM patients were classified as having the worst QOL, worse than for inflammatory disease patients (i.e. RA, SpA and SLE); such findings were consistent with previous studies in Colombia.27,28 HRQOL has been assessed on 171 FM patients in the USA using EQ-5D-31.
