2022, Orthopaedics and Traumatology: Surgery and ResearchCitation Excerpt :Bucket-handle meniscal tear (BHMT) may be isolated or associated with anterior cruciate ligament (ACL) sprain, and accounts for 10% of meniscal lesions [1]. The risk of medial meniscal lesion increases with the age of the ACL tear and of the patient [2,3], and lateral BHMT is usually associated with or concomitant to acute ACL trauma [4]. Conservative treatment is advised, reducing the displaced part into an anatomic position then suturing it [5–7], as meniscectomy in so extensive a lesion would involve ablating most of the meniscus [8–11].

Gadeyne et al. stated that severity and incidence of lesions to the lateral meniscus appear to be unaffected by the time to surgery after trauma.6 Similarly, Michalitsis et al. revealed that time from injury to operative intervention was not a significant factor for the presence of a meniscal lesion.7

[26], in a human cadaver model, reported 58% increase in anterior drawer on Lachman maneuver in case of medial meniscal tear associated with ACL sectioning; they suggested that the posterior segment of the medial meniscus plays a role in anteroposterior stabilization of the tibia. Gadeyne et al. [27] reported that medial meniscus tear rates increase with age and accident-to-surgery time. In the lateral meniscus, Hosseini et al. [28] showed that associated ACL tear increased anterior tibial subluxation by 2.5 mm compared to the contralateral knee when climbing stairs; ACL tear associated with medial meniscus tear, on the other hand, did not affect anterior subluxation.
