Bronchopulmonary Dysplasia

Bronchopulmonary dysplasia has become the most common pulmonary sequelae in neonates receiving mechanical ventilation. The pathogenesis of BPD is multifactorial, but prematurity, positive pressure ventilation, oxygen toxicity and pulmonary edema are some of the most important factors in its development. By minimizing these factors, it is possible to reduce the incidence and severity of BPD.

      Nearly two thirds of those born at < 29 weeks' gestation required mechanical ventilation during the newborn period.5 Prolonged mechanical ventilation and endotracheal intubation in premature infants, however, is associated with both adverse neurodevelopmental outcomes and acute complications, and long-term sequelae, such as plugged endotracheal tube, pneumonia, pneumothorax, subglottic stenosis, and bronchopulmonary dysplasia (BPD).6,7 Therefore, extubation of ventilated infants as early as possible is the clinical goal.

      On histologic samples, the characteristic areas of hyperinflation alternating with areas of focal collapse were often noted, and hyperplasia of the bronchial epithelium.8 Radiography of these infants showed areas of heterogeneity throughout the lung fields and coarse scattered opacities in the most severely affected of infants.9 The classical BPD described by Northway and colleagues has been replaced by a milder form of the disease.
