Medical interventions are offered to individuals with the aim of diagnosing or treating illnesses or improving health. Interventions may lead to negative experiences e.g., anxiety and/or pain, and individuals may need to carry out specific behaviors to aid the running of, or recovery from, medical interventions. This article considers the role of psychology in preparing people for medical interventions. Individuals may require support in decision making around whether or not to accept an offered intervention. Preparation may then ensure that anxiety and pain are minimized and managed effectively, optimize post-procedure outcomes, and support the performance of recommended or required behaviors.

Positive effects of providing targeted procedural preparation prior to medical procedures have been studied across several pediatric clinical settings including: orthopedic cast removal [46], angiocatheter insertion [47], bone-marrow aspirations and lumbar punctures [48]. Similarly in adult settings, studies have examined this in surgical patients [49], cholecystectomy surgery [50], and endoscopic procedures [51]. A positive effect was achieved in reducing pain intensity with use of the Ditto™ (p = 0.051).
The relationships between pain, stress and anxiety, and their effect on burn wound re-epithelialization have not been well explored to-date. The aim of this study was to investigate the effect of the Ditto™ (a hand-held electronic medical device providing procedural preparation and distraction) intervention on re-epithelialization rates in acute pediatric burns.
From August 2011 to August 2012, children (4–12 years) with an acute burn presenting to the Royal Children's Hospital, Brisbane, Australia fulfilled the study requirements and were randomized to [1] Ditto™ intervention or [2] standard practice. Burn re-epithelialization, pain intensity, anxiety and stress measures were obtained at every dressing change until complete wound re-epithelialization.
One hundred and seventeen children were randomized and 75 children were analyzed (n = 40 standard group; n = 35 Ditto™ group). Inability to predict wound management resulted in 42 participants no longer meeting the eligibility criteria. Wounds in the Ditto™ intervention group re-epithelialized faster than the standard practice group (−2.14 days (CI: −4.38 to 0.10), p-value = 0.061), and significantly faster when analyses were adjusted for mean burn depth (−2.26 days (CI: −4.48 to −0.04), p-value = 0.046). Following procedural preparation at the first change of dressing, the Ditto™ group reported lower pain intensity scores (−0.64 (CI: −1.28, 0.01) p = 0.052) and lower anxiety ratings (−1.79 (CI: −3.59, 0.01) p = 0.051). At the second and third dressing removals average pain (FPS-R and FLACC) and anxiety scores (VAS-A) were at least one point lower when Ditto™ intervention was received.
The Ditto™ procedural preparation and distraction device is a useful tool alongside pharmacological intervention to improve the rate of burn re-epithelialization and manage pain and anxiety during burn wound care procedures.
Sensations during removal of epicardial pacing wires after coronary artery bypass graft surgery

One procedure that is commonly performed after coronary artery bypass graft surgery is removal of epicardial pacing wires (EPWs). Currently, there is a paucity of information regarding the sensations experienced by patients during this procedure.
To determine the sensations commonly reported by patients during EPW removal.
A secondary analysis was performed on data sets from 2 previous studies to determine the sensations commonly experienced by patients during EPW removal. The sample consisted of 144 subjects from 2 teaching hospitals.
The most commonly reported sensation during EPW removal was a “mild to moderate pulling” sensation.
This secondary analysis provides important information regarding the sensations most commonly reported by patients during EPW removal. This sensory information could be incorporated into a psycho-educational information intervention to prepare patients for EPW removal.
The effect of an information brochure on patients undergoing gastrointestinal endoscopy: A randomized controlled study

Optimal information provision is all the more important since the recent established patients’ legal right – in many countries – to be informed fully and in advance about a medical examination, and to give their consent on the basis of this information. Earlier studies in other countries showed that information has a positive effect on undergoing a gastrointestinal endoscopy [4,5], although these effects were sometimes small [6]. Positive effects were also found for additional behavioral and cognitive instructions [7,8], and, in case of colonoscopy, for self-selected music [9].
The aim of the present study was to test the potential beneficial effects of an information brochure on undergoing a gastrointestinal endoscopy for the first time. The information provided was based on self-regulation theory, and patients could restrict themselves to reading the summary only.
Patients were randomly assigned to an experimental group receiving the brochure at least 1 day before the gastroscopy (N = 47), or to a control group not receiving the brochure (N = 48).
The results show that all experimental subjects, except one, fully read the brochure. Those receiving the brochure experienced less anxiety before the gastroscopy and, afterwards, they reported greater satisfaction with the preparation for it. With regard to coping style there were some small moderating effects into the direction expected: low blunters (those not seeking distraction under impending threat) as compared to high blunters showed extra reduced anxiety after reading the brochure. They also tended to read the brochure more often. High monitors (those seeking information under impending threat) receiving the brochure showed reduced anxiety during the gastroscopy as compared to low monitors (tendency).
We conclude that providing patients with the developed brochure constitutes an efficient, beneficial intervention.
The brochure could easily be implemented in standard practice without the necessity to take the patient's coping style into account.
