Further evidence for the validity of the beck anxiety inventory with psychiatric outpatients

Abstract

To provide further information about the psychometric characteristics of the Beck Anxiety Inventory (BAI), the BAI was administered to 470 outpatients with mixed psychiatric disorders along with the revised Beck Depression Inventory (BDI) and the SCL-90-R. The BAI's internal consistency was high (alpha = .92), and it was significantly more correlated with the SCL-90-R Anxiety subscale (r = .81) than it was with SCL-90-R Depression subscale (r = .62). However, it was also significantly correlated with the BDI (r = .61). The mean BAI scores of the 141 (30.0%) outpatients with mood disorders and the 86 (18.3%) outpatients with anxiety disorders were comparable, but higher than the mean BAI score of the 243 (51.7%) outpatients with other disorders. A principal components analysis found the previously reported subjective, neurophysiological, panic, and autonomic dimensions. The results are discussed as providing further evidence for the use of the BAI with psychiatric outpatients.

Towards better video services: An EEG-based interpretable model for functional quality of experience evaluation

Since emerging video services can provide emotional and social value to users, the setting of their functional parameters directly affects human cognitive and affective states, further influencing video services’ quality of experience (QoE), which we call functional QoE (fQoE). FQoE is highly dependent on human subjective perceptions and the reasons for its generation are important for service providers to optimize video services. However, existing fQoE research methods are unable to perform quantitative assessment and lack interpretability. Electroencephalogram (EEG) signals have the advantage of being difficult to disguise, and contain rich brain activity information, gaining more attention from researchers nowadays. Based on EEG, we propose an interpretable model to evaluate fQoE, and the model is tested on a self-built dataset for bullet chatting video (BCV) service. Our model can effectively fuse single electrode and multi-electrode features from EEG, and introduces Graph-based Brain-area Perception Network (GBPN) for extracting fQoE sensitive brain areas, achieving satisfactory results. We find brain areas associated with fQoE caused by different functional parameters of BCV. To sum up, our fQoE model enables quantitative assessment with neurophysiological interpretability of fQoE, providing a scientific basis for the optimization and development of video services.
The psychological and physiological effects of integrated cognitive-behavioral and biofeedback therapy on panic disorder: A randomized controlled trial

Cognitive-behavioral therapy (CBT) and biofeedback therapy are commonly regarded as effective treatment modalities for panic disorder. The aim of this study was to establish a Taiwanese version of an integrated cognitive-behavioral and biofeedback therapy (ICB) and examine its effects on panic disorder using psychological and physiological indicators.
Thirty patients with panic disorder were enrolled in this study. They were randomly assigned to either the ICB group (n = 15) or the treatment as usual (TAU) group (n = 15). The intervention consisted of six sessions, conducted once a week. Psychological indicators were measured at baseline (prior to intervention), week 3, and week 6, while physiological indicators were measured at baseline and week 6. The psychological indicators included five scales, with the Panic Disorder Severity Scale (PDSS) being the primary measure. The physiological indicators included respiratory sinus arrhythmia (RSA) and skin conductance, which respectively represent parasympathetic and sympathetic activity.
Considering all participants, PDSS scores significantly decreased over time, but the difference between the ICB and TAU groups did not reach statistical significance. Among the physiological indicators, resting-state RSA and RSA under relaxation showed significant between-group differences over time, with the ICB group demonstrating a more pronounced improvement in RSA.
In the context of existing pharmacological treatments, the benefits of ICB for panic disorder may not be observable through psychological indicators. However, it can lead to enhancement of parasympathetic activity as evidenced by the physiological indicators.
Impaired action-safety learning and excessive relief during avoidance in patients with anxiety disorders

Anxiety-related disorders are characterized by high levels of avoidance, but experimental research into avoidance learning in patients is scarce. To fill this gap, we compared healthy controls (HC, n = 47) with patients with obsessive-compulsive disorder (OCD, n = 33), panic disorder with agoraphobia (PDA, n = 40), and post-traumatic stress disorder (PTSD, n = 66) in a computer-based avoidance learning task, in order to examine (1) differences in rates of avoidance responses, (2) differences in action-safety learning during avoidance, and (3) differences in subjective relief following successful avoidance. The task comprised aversive negative pictures (unconditional stimulus, US) that followed pictures of two colored lamps (conditional stimuli, CS+), but not a third colored lamp (safety stimulus, CS-), and could be avoided by pressing a button during one CS+ (CS+ avoidable) but not the other (CS+ unavoidable). Participants rated their US-expectancy and level of relief on a trial-by-trial basis. Compared to the HC group, patient groups displayed higher levels of avoidance to the safety stimulus, and higher levels of US-expectancy and relief following the safety and avoidable danger stimulus. We propose that patients with anxiety disorders have low confidence in the safety consequences of avoidance actions, which induces increased relief during US omissions that reinforce the avoidance action.
Barriers to Access to Mental Health Services in Pregnant Women With Mental Pathology Residing in Colombia

Pregnancy is associated with multiple physiological changes. Anxiety, depression, suicide, and consumption of psychoactive substances during gestation have been phenomena rarely studied by Colombian academia. These disorders are among the principal causes of maternal morbidity–mortality; however, the presence of barriers to care constitutes a public health problem that must be studied.
A prospective longitudinal observational descriptive study was conducted, during one year, with 166 pregnant women. The prevalence of these diseases was evaluated and the most significant supply and demand barriers were identified, for which a battery of mental health measurement instruments was applied to identify the risk of consuming psychoactive substances, depression, and anxiety.
The most-frequent mental pathology was gestational depression, occurring in 57.22% of the sample; the second most-frequent pathology was gestational anxiety, diagnosed in 46.98% of the patients. The disorder due to consumption of most-frequent substances was smoking; followed by cannabis and alcohol. Access barriers, both in supply and demand, were present in most of the population; 53.01% of the pregnant women were not evaluated by psychology. The principal supply barrier was not assigning appointments or, failing that, these were assigned for a very distant date.
A clear relationship exists among depression and anxiety, number of children and age, and supply barriers and access to specialized mental health care. Our findings suggest a high frequency of access barriers in the gestating population with mental pathology.
El embarazo se asocia con cambios fisiológicos. La ansiedad, la depresión, el suicidio y el consumo de sustancias psicoactivas durante la gestación han sido fenómenos escasamente estudiados en Colombia. Estos trastornos son una de las principales causas de morbimortalidad materna; sin embargo, la presencia de barreras para la atención, constituyen una problemática de salud pública que debe ser estudiada.
Se ejecutó un estudio descriptivo observacional longitudinal prospectivo, durante un año, con 166 gestantes. Se evaluó la prevalencia de estas enfermedades, y se identificaron las barreras de oferta y demanda más significativas, para ello se aplicó una batería de instrumentos de medición de salud mental para identificar el riesgo de consumo de sustancias psicoactivas, depresión y ansiedad.
La depresión gestacional ocurrió en el 57,22% de la muestra; la ansiedad gestacional se presentó en el 46,98% de las pacientes. El trastorno por consumo de sustancias más frecuentes fue el tabaquismo; seguido del cannabis y el alcohol. Se presentaron barreras de acceso en la mayoría de la población. El 53,01% de las gestantes no fueron valoradas por psicología. La principal barrera de oferta fue el no agendamiento de citas.
Existe una relación entre depresión y ansiedad, número de hijos y edad, y barreras de oferta y acceso a atención especializada en salud mental. Nuestros hallazgos sugieren una alta frecuencia de barreras de acceso en la población gestante con enfermedad mental.
Anxiety predicts reduced weight loss 30 months after bariatric surgery

Though psychosocial factors are routinely examined in presurgical psychological evaluations, the predictive value of some psychosocial factors on postsurgical weight loss is still relatively unknown. Additional research examining the predictive value of psychological constructs preoperatively and long-term weight outcomes is needed to enhance the clinical utility of the presurgical psychological evaluations.
This study aimed to examine psychosocial factors as predictors of weight outcomes 30 months after bariatric surgery.
University hospital in the Appalachian region of United States.
Participants included 196 adults who underwent bariatric surgery. Psychosocial data were collected as part of a routine psychological evaluation prior to bariatric surgery. Objective weight was obtained through patients’ medical records at 12, 24, and 30 months after surgery.
Linear mixed models was used to examine presurgical psychosocial factors predicting postsurgical weight loss (n at 12 months = 153, n at 24 months = 130, n at 30 months = 92). Anxiety had a significant interaction effect with time (estimate = –.01, P = .013), indicating that higher anxiety was associated with less weight loss over time. Those with severe anxiety before surgery lost the most weight 12 months after surgery but also regained the most weight 30 months after surgery. Other predictors were not statistically significant.
Anxiety assessed prior to surgery predicted reduced weight loss 30 months after bariatric surgery, after controlling for surgery type, baseline weight, sex, and age. Results highlight the importance of evaluation and treatment of anxiety in presurgical bariatric candidates.
The 9-year clinical course of depressive and anxiety disorders: New NESDA findings

In longitudinal research, switching between diagnoses should be considered when examining patients with depression and anxiety. We investigated course trajectories of affective disorders over a nine-year period, comparing a categorical approach using diagnoses to a dimensional approach using symptom severity.
Patients with a current depressive and/or anxiety disorder at baseline (N = 1701) were selected from the Netherlands Study of Depression and Anxiety (NESDA). Using psychiatric diagnoses, we described ‘consistently recovered,’ ‘intermittently recovered,' ‘intermittently recurrent’, and ‘consistently chronic’ at two-, four-, six-, and nine-year follow-up. Additionally, latent class growth analysis (LCGA) using depressive, anxiety, fear, and worry symptom severity scores was used to identify distinct classes.
Considering the categorical approach, 8.5% were chronic, 32.9% were intermittently recurrent, 37.6% were intermittently recovered, and 21.0% remained consistently recovered from any affective disorder at nine-year follow-up. In the dimensional approach, 66.6% were chronic, 25.9% showed partial recovery, and 7.6% had recovered.
30.6% of patients were lost to follow-up. Diagnoses were rated by the interviewer and questionnaires were completed by the participant.
Using diagnoses alone as discrete categories to describe clinical course fails to fully capture the persistence of affective symptoms that were observed when using a dimensional approach. The enduring, fluctuating presence of subthreshold affective symptoms likely predisposes patients to frequent relapse. The commonness of subthreshold symptoms and their adverse impact on long-term prognoses deserve continuous clinical attention in mental health care as well further research.
