Squamous odontogenic tumorlike proliferations in odontogenic cysts

Abstract

Five cases of squamous odontogenic tumorlike proliferations occurring in the walls of odontogenic cysts are presented. These lesions showed no more aggressive potential than the cysts, if taken alone, in which they occurred. The lesions showed no tendency to develop into solid tumors and, on the basis of this limited series of cases, conservative surgery appears to be the treatment of choice.

SOT-like proliferations are considered a reactive proliferation either of the cystic epithelium or of residual odontogenic rests present in the wall of the cyst.44,45 The islands seldom form microcysts or display individual cell keratinization as seen in SOT.

    The radicular cysts selected for this study were defined as tissue composed of fibrous connective tissue and/or granulation tissue infiltrated by acute and/or chronic inflammatory cells exhibiting evidence of either an epithelial lining or a cystic lumen lined by nonkeratinized stratified squamous epithelium (15). In selecting cases of SOTLPs in radicular cysts, we used the microscopic criteria set forth by Wright (2), namely an odontogenic cyst containing multiple nonkeratinized squamous epithelial islands in the fibrous cyst wall that are morphologically identical to the SOT. In every case, light microscopic examination of sections stained with hematoxylin-eosin verified the presence of the SOTLP component.
