Abstract

A case of a 1-year-old boy with combined transposition of the great arteries and total anomalous pulmonary venous return is reported. The anomalies were successfully repaired by a new atrial partitioning technique.

Mustard Baffle Revision With Systemic Ventricular Assist Device Placement

Systemic right ventricle failure often develops in patients with dextro-transposition of the great arteries who have undergone atrial-level switch procedures. An adult with dextro-transposition of the great arteries with Mustard repair in childhood presented with end-stage heart failure due to systemic right ventricle failure, Mustard baffle leak, and unrepaired partial anomalous pulmonary venous return. She underwent device closure of the baffle leak by catheterization, followed by baffle revision and concomitant systemic ventricular assist device placement by adult and pediatric heart surgeons. This case demonstrates the importance of multidisciplinary collaboration between pediatric and adult heart failure specialists in caring for complex adult congenital heart patients.

The dual drainage of the common chamber was again a unique presentation, but it did not alter the surgical technique. The association of TGA with TAPVC allows oxygenated blood to drain into the RA and then into the systemic circulation, thus avoiding significant desaturation and pulmonary overflow [1–3]. This is why this condition in these babies may go undetected for longer periods, delaying an ASO.
Transposition of the great arteries (TGA) with total anomalous pulmonary venous connection (TAPVC) is a rare association. Very few such cases have been reported. Among them 1 patient underwent anatomic repair. The rest received repair at the atrial level with either a modified Senning technique or a Mustard technique. We report a rare combination of TGA, TAPVC draining to the coronary sinus, and left juxtaposition of the atrial appendages with a diminutive right atrium. The anomalies were successfully repaired by anatomic correction.

Partial or total anomalous pulmonary venous return can also occur and must be excluded in the evaluation because this anomaly would likely be repaired at the same time as the primary surgical repair for TGA.27 On further imaging of the atria, the atrial appendages should be identified.
