What have we learned in the last 20 years? A comparison of a modern era pediatric and congenital catheter ablation registry to previous pediatric ablation registries

In pediatric cardiology alone, there are multiple efforts to improve outcomes, ranging from the Congenital Database of the Society of Thoracic Surgeons to PediMacs, the pediatric ventricular assist device registry.6,7 The development of the MAP-IT registry in 2014 allowed pediatric electrophysiologists an infrastructure to measure patient-centered outcomes and complications.2

Postoperative multiorgan failure is the final common pathway for inadequate systemic perfusion, either prolonged or acute.300,301 At risk are children with preoperative end organ dysfunction; single-ventricle physiology after the Norwood operation; systemic or pulmonary ductal dependent lesions following a systemic-to-pulmonary or pulmonary-to-systemic shunt; TOF after complete repair with right ventricular dysfunction; TGA with pulmonary hypertension; anomalous left coronary artery arising from the pulmonary artery after repair with left ventricular dysfunction; failed single-ventricle palliation after heart transplantation; chromosomal abnormalities; and protracted postoperative arrhythmias related to an atrial reentry circuit (atrial flutter and atrial fibrillation), atrial automaticity (atrial ectopic tachycardia), junctional automaticity (accelerated junctional rhythm or JET), AV reentry, and ventricular arrhythmias.
