2019, Cardiac Electrophysiology ClinicsCitation Excerpt :Surface ECGs may sometimes be unreliable in diagnosing true LBBB with an activation delay, as demonstrated by invasive electrophysiologic studies.19,20 Approximately one-third of patients are misdiagnosed as LBBB just by the present ECG criteria and have more than 1 septal breakthrough site indicating a lack of true activation delay.19,21 A stricter definition to diagnose LBBB by ECG has been proposed by Straus and colleagues,22 with 2 specific changes to distinguish it from interventricular conduction delays: (1) longer QRS duration—greater than or equal to 130 ms in women and greater than or equal to 140 ms in men and (2) mid-QRS notching in 2 contiguous leads.
