Cardiovasculaire streefwaarden ouderen verschillen per patiënt

Samenvatting

In 2013 is een stroomdiagram gepubliceerd met individuele streefwaarden voor cardiovasculair risicomanagement specifiek voor ouderen, op basis van patiëntkarakteristieken. Dit diagram zou als leidraad kunnen dienen voor de praktijkondersteuner. Door het gebruiken van dit diagram kun je helpen voorkomen dat voor kwetsbare ouderen te lage streefwaarden gehanteerd worden. De complexiteit van de streefwaarden voor ouderen is een bijzondere uitdaging voor de praktijkondersteuner, vooral in de communicatie.

Leefstijladviezen over roken, gewicht en lichaamsbeweging zijn ook bij ouderen het belangrijkste aspect bij de preventie van hart- en vaatziekte. Bij ouderen pas je adviezen over beweging aan hun mogelijkheden aan. Bij voeding besteed je speciaal aandacht aan zout.
