Een elektrocardiogram bij elke patiënt met hypertensie

Doel Een aantal bestaande richtlijnen adviseert om bij hypertensiepatiënten een elektrocardiogram te maken om het cardiovasculaire risico nauwkeuriger te kunnen schatten. We onderzochten de prevalentie van klinisch relevante ecg-afwijkingen in deze patiëntengroep en maakten een schatting van het number needed to screen (NNS): het aantal ecg’s dat men bij hypertensiepatiënten moet maken om bij adequate behandeling gedurende tien jaar één sterfgeval te voorkomen.
Methode De onderzoekspopulatie bestond uit 866 hypertensiepatiënten van het Leidsche Rijn Gezondheidsproject (een cohortonderzoek in Leidsche Rijn bij Utrecht). Met behulp van de SCORE-tabel (Systematic COronary Risk Evaluation) hebben we bij hen alle risicofactoren gemeten die nodig zijn voor een risicoschatting, waarna we een ecg maakten. We interpreteerden dit met behulp van het Modular ECG Analysis System (MEANS). We berekenden het NNS om één sterfgeval te voorkomen met behulp van de inverse prevalentie (1/prevalentie) van ecg-afwijkingen, vermenigvuldigd met het number needed to treat (NNT): het aantal patiënten dat men moet behandelen om één sterfgeval te voorkomen door behandeling van de ecg-afwijkingen conform bestaande richtlijnen.
Resultaten De populatie bestond voor 54,2% uit mannen en was gemiddeld 53,2 jaar oud (sd 11,5). De prevalentie van ecg-afwijkingen was 17,6% (95%-BI 15,0-20,1). Atriumfibrilleren en myocardinfarct kwamen voor bij 2,1% (95%-BI 1,1-3,0). De prevalentie van andere ecg-afwijkingen die de kans op hart- en vaatziekten (HVZ) verhogen was 15,4% (95%-BI 13,1-17,9). Het NNS om één sterfgeval door HVZ in tien jaar te voorkomen schatten we op 260 (95%-BI 220-308).
Conclusie Deze bevindingen ondersteunen richtlijnen die aanbevelen om een ecg te maken bij hypertensiepatiënten. De prevalentie van klinisch relevante afwijkingen is namelijk aanzienlijk en het NNS om één sterfgeval te voorkomen is lager dan bij andere algemeen geaccepteerde tests.
