Bloeddruk meten buiten de spreekkamer: de kloof tussen praktijk en wetenschap

Bloeddrukmeting is een vaardigheid waarop in de huisartsenpraktijk vaak een beroep wordt gedaan, veelal om het cardiovasculair risico te bepalen of ter controle van een behandeling. Auscultatoire bloeddrukmetingen maken nog steeds het leeuwendeel van de metingen uit, maar de oscillometrische meettechniek is de laatste tien jaar sterk in opkomst, zowel in de huisartsenpraktijk als thuis. In dit artikel bespreken we de stand van zaken met betrekking tot bloeddrukmeting en de indicaties voor thuis- en ambulante metingen van de bloeddruk.
Thuismetingen liggen gemiddeld 5 mmHg lager dan metingen in de huisartsenpraktijk. De belangrijkste indicatie voor zowel thuis- als ambulante metingen is het vaststellen van een mogelijk aanwezig wittejasseneffect. Men kan de SCORE-tabel uit de NHG-Standaard Cardiovasculair risicomanagement gebruiken bij de interpretatie van bloeddrukmetingen in de huisartsenpraktijk, maar niet bij thuismetingen.
